Biceps Brachii

Long head origin attaches to supraglenoid tubercle. Insertion at radial tuberosity. Short head origin attaches to coracoid process. Insertion of short head at bicipital aponeurosis of elbow.

Prime flexor of the shoulder and elbow, and supinates

Nerve serving the biceps is the musculocutaneous nerve - C6

Fusiform Muscle

Brachialis

Origin found at anterior shaft of humerus working down to insertion at ulnar

Flexes the elbow

Served by the musculocutaneous nerve - C6 

Triceps

Made up of Three heads:

- Long head origin attaches to inferior glenoid tubercle

- Medial and short head origin attach to posterior shaft of humerus.

- Insertion to the olecranon of ulnar

Served by Radial nerve - C7

Origin attaches at lower supercondylar ridge of humerus. Insertion to the distal, lateral border of the radius

Flexes Elbow

Served by Radial Nerve - C6

Extensor carpi radialis longus

The origin attachment is at the the distal, lateral supracondylar ridge. Insertion at base of 2nd metacarpal.

Supplied by radial nerve - C6-7

The origin attahcment is at the lateral epicondyle of humerus. Insertion at base of 3rd metacarpal.

Supplied by deep branch of radial nerve - C6-8

Origin at lateral epicondyle of humerus. Insertion is at the distal end of phalanges via dorsal mechanism of digits 2-5.

Supplied by deep branch of radial nerve - C6-8

Origin at lateral epicondyle of humerus. Insertion attachment at base of 5th metacarpal 

Origin attachment at medial epicondyle of humerus. The insertion at base of both the second and third metacarpals. 

Supplied by median nerve - C6-7

Origin at medial epicondyle of humerus. Insertion attachment at flexor retinaculum and palmar aponeurosis. 

Supplied by median nerve - C7-8

Origin at medial epicondyle of humerus. Insertion attachment at pisiform, hook of hamate and at base of 5th metacarpal.

Supplied by ulnar nerve - C8, T1

Flexor carpi digitorum superficialis 

Origin at humero-ulnar head: positioned at medial epicondyle of humerus and coronoid process of ulna. Also at radial head: upper anterior surface of radius. Insertion to middle phalanges digits 2-5.

Supplied by median nerve - C7-8, T1

Origin at anterior surface of first 8/9 ribs. Insertion at Anterior surface of medial border of scapula.

Supplied by thoracic nerve - C5-C7

Pectoralis minor

Origin at anterior surface of the sternal ends of first 3-5 ribs. Insertion at coracoid process of scapula.

Supplied by medial pectoral nerve - C8, T1

Origin attachment on the transverse processes of the cervical spine - C1 to C4. Insertion at medial border of scapula between spine and superior angle.

Supplied by Dorsal scapular nerve - C5

Also, by Ventral primary rami of C3-C4.

Origin at medial superior nuchal line and external protuberance & occipitical bone, ligamentum nuchae & spinous process of C7- T12. Insertion at Lateral clavicle, acromion and spine of scapula, known as a Triangular muscle. 

Spinal segment - C3-4 and accessory cranial nerve XI

Origin spinous process of T2-T5. Insertion at medial border inferior to spine of scapula. 

Supplied by dorsal nerve - C5

Origin at spinous process C7-T1. Insertion at medial aspect of scapula, superior to spine.

Supplied by dorsal nerve - C5

origin at angle of ribs 3 to 6. Insertion at transverse process of C4-C6.

Supplied by dorsal rami of spinal nerves - C6-8

Origin at angles of ribs 7 to 12, medial to insertions of iliocostalis lumborum. Insertion at angles of ribs 1 to 6; transverse process of C7.

Supplied by dorsal rami and thoracic spinal nerves

Origin attachment at surfaces of ribs 5 to 12. Insertion at Anterior illiac crest & abdominal aponeurosis {lateral to abdomen} to linea alba {centre of abdomen}.

Nerve supplied by intercostal, iliohygastric & ilioinguinal nerves.

Origin attachment at anterior illiac crest, lateral half of inguinal ligament and thoracolumbar fascia. Insertion costal cartilages of ribs 8-12; abdominal aponeurosis to line alba.

Nerve supplied by intercostal, iliohygastric, ilioinguinal nerve.

Origin attachment at anterior illiac crest, lateral half of inguinal ligament, thoracumbular fascia & cartilages of ribs 6-12. Insertion at abdominal aponeurosis to linea alba, xiphoid process & pubic symphysis.

Supplied by 7-12 intercostal, iliohygastric & ilioinguinal nerves.

Origin attachent and pubic crest and symohysis pubis. Insertion attachment at costal cartilages of ribs 5-7; xiphoid process of sternum. 

Supplied by intercostal nerve 7-12

Origin attachment of sternal head at sternum, upper costal cartilages (1-6) and the aponeurosis of external oblique.

Orgin of sternal head at sternum, aponeurosis of external oblique and upper costal cartilages. Insertion attachment at crest of greater tubercle of the humerus.

Origin of clavicular head at medial half of clavicle. Insertion at lateral lip of bicipital groove of humerus.

Supplied by lateral and medial pectoral nerve - C5-8, T1
