Term

What is shrinker?

How often should one be worn?

Definition

  • A garment that goes onto the leg that is designed to help the limb ‘mature’
  • The shrinker provides graded compression
  • The shrinker should be worn constantly, except for during the time that the prosthesis is being worn.

Term
What is meant by "limb maturation" and what occurs intitally after a limb has been amputated?
Definition
  • Reduce in volume
  • Desensitize - the shrinker being total contact will help to de-sensitize the limb
  • Form into a conical shape - this is so that it can fit will inside a prosthesis
  • After the limb has been cut, it must go through a very major healing process, it has to recreate its own vascular system
Term
What is the purpose of prosthetic sox?
Definition
  • Decrease sheer forces
  • Provide cushion
  • Absorb moisture

a nylon garment that is worn to

protect the skin under a prosthesis

Term
What is the purpose of the "socket"
Definition
  • Transfer body weight to the prosthesis
Term

What are the WB tolerant areas of

a PTB (patella tendon bearing socket)

Definition
  • Patellar tendon
  • Shaft of the Fibula
  • Anterior Compartment
  • Medial Flair of Tibia
  • Gastroc

  • Popteal area

Term

What are the non-WB tolerant areas that are pressure sensitive to for the PTB

(patellar tendon bearing) socket

Definition
  • Fib Head
  • Anterior Distal Tibia
  • Crest of Tibia
  • Peroneal Nerve
  • Cut-end of the fibula
  • Lateral Tibial Condyle
  • Hamstring Insertion Points (medial and lateral)
Term
What type of socket is a TSB socket?
Definition
  • Total Surface Bearing socket
  • Allows for total contact, and
  • WB over the entire surface of the limb
Term

What is the purpose of a

socket insert, a.k.a. socket liner?

Definition
  • Can help prevent scar tissue formation
  • Makes adjustments to the prostetic easier and more comfortable
Term

What are advantages and disadvantages to a transtibial liner?

Definition

Advantages

  • Ease of adjustment
  • Soft WB
  • Allows for le-way in sock-ply error
  • Disadvantages

  • Added bulk and weight to a prostetic limb
  • Durability (they can rip easily)
  • Hygiene
  • Term

What types of suspension can be used

    for a transtibial prosthesis

    Definition
    • Supracondylar - cuff suspension
    • Suction with sleeve (harmony)
    • Sleeve
    • Pin lock
    • Joint and thigh lacer

    Term
What is the difference between a temporary and diffinitive prosthesis?
    Definition
    • Temporary: made of a poly-propoline plastic  ( a thermal plastic) it can be heated and reshaped
    • Definitive: placed in ~6 months after wearing the temporary (once they are well fitted and adjusted to their device)
    Term

    Prosthetic Feet

What is the difference between a:

    • Articulated
    • Non-articulated
    • SACH
    • Running or J shaped foot
    Definition
    • Articulated - contains joints, can be single axis or multiaxis, and have dynmaic responses
    • Has a heal lever and toe level, allowing for greater PF, DF
    • They are made based on activity level and body weight
    • But they are heavier and have more maintenance issues
    • Non-articulated - has no joints
    • SACH (soft arch cushion heel)- very light weight, durable, and inexpensive
    • Running foot or J shaped foot - has no heel
    Term
What is the purpose of a keel?
    Definition
    • Creates a dynamic response by storing and releasing energy at the forefoot
    • Gives some integrity by creating a lever arm
    Term
What is the purpose of a prosthetic shrinker
    Definition
    • reduce edema
    • reduce phantom pain (de-sensitize)
    • promote a cylindrical shape
    Term
Why is a transtibial socket aligned in flexion?
    Definition
    • to increase the WB surface on the residual limb
    Term
When gait training with a transtibial  amputee what motion  at the knee is important during heel strike?
    Definition
    a flexion moment
    Term

A new patient presents with knee

    hyperextension during gait training,

how do you correct this problem?

    Definition
    • Increase the socket flexion and/or move the foot posterior
    • Increase the heel height of the shoe and/or increase heel stiffness of the shoe/foot
    • Send back to the prosthetist
    Term
If a new patient presents with too much knee flexion during gait training what is one way that you could correct this problem?
    Definition
    increase the toe lever (keel)
    Term
What are three main objectives that should be taught to a transtibial when gait training?
    Definition
    • Weight shifting over the prosthesis
    • Speding equal time on each limb throughout the gait cycle
    • Not to look down, but to look up
    Term
At heel off where are the GRFs that will cause a knee extension moment?
    Definition
    Anterior to the knee
