t-PA (Alteplase) 

100 mg IV over 90 minutes

• 15 mg bolus followed by 0.75 mg/kg infusion (not to exceed 50 mg) over 30 minutes 
•  followed by 0.5 mg/kg infusion (not to exceed 35 mg) over 1 hour.

• 10 IU bolus IV over 2 minutes followed 30 minutes later with another 10 IU bolus IV

60 U/kg  IV bolus followed by 12 U/kg/hr

at the time rt-PA or rPA therapy is initiated adjust to maintain aPTT at 1.5-2.0 times control

no IV bolus of heparin and delay infusion 6 hrs or until APTT returns to less than 2 times control with Streptokinase, anistreplase, urokinase

30 mg IV bolus, followed by 1 mg/kg SQ q12h

2.5 mg daily
