Term

Stroke

Definition

A new focal neurological deficit lasting for more than 24 hours and presumed to be vascular in origin.

Term

What are the categories of stroke and what is an example of each?

Definition

Ischemic stroke (cerebral infarction)

Hemorrhagic stroke (intracerebral hemorrhage)

Term
List the causes of epidemiology of ischemic strokes and at what percentage they occur.
Definition

Atherosclerotic (large vessel disease) - 20%

Small vessel disease - 25%

Cardiogenic embolism - 20%

Cryptogenic - 30%

Other causes - 5%

Term
What are non-modifiable risk factors of stroke?
Definition

Age - risk increases with increasing age

Gender - males > females

Race - blacks>hispanic>white>asians

Ethnicity

Genetics

Term
What are the risk factors for a hemorrhagic stroke?
Definition
  • HTN
  • smoking (2-4x increased risk)
  • drug abuse (cocaine, amphetamines, alcohol)
  • arteriovenous malformations
  • cranial tumors
  • anticoagulation
  • trauma
Term
What are the risk factors for ischemic stroke?
Definition
  • non-modifiable risk factors (age, gender, race)
  • low birth weight
  • family history of stroke or TIA
  • CVD (CHD, heart failure, peripheral artery disease)
  • HTN
  • smoking or exposure to smoking
  • DM
  • asymptomatic caroticd stenosis
  • dislipidemia
Term
What are the risk factors for cardioembolic stroke?
Definition
  • atrial fibrilation
  • valvular disease
  • acute MI with LV thrombosis
  • cardiomyopathy
  • prosthetic heart valve
  • infective endocarditis
Term

HYVET

Definition

Population: >80yo with isolated SBP > 160mmHg

Intervention: reduction of SBP with medication

Comparison: Thiazide +/- ACEI vs. placebo

Outcome:

A 30% reduction of stroke was seen in the treatment arm compared to the placebo; note that the SBP was lowered by 10mmHg on average

Term

Syst-Eur

SHEP

Definition

Population: Elderly w/isolated HTN;

SBP>160, DBP<90

Intervention: reduction of SBP with medication

Comparison: CCB or Thiazide +/- BB vs. placebo

Outcome:

Syst-Eur showed a 42% reduction in stroke

SHEP showed a 36% reduction in stroke

Term
