Term
What are the six extrinsic eye muscles?
Definition
Superior rectus, inferior rectus, medial rectus, lateral rectus, superior oblique, and inferior oblique.
Term
What is a Strabismus?
Definition
A misalignment or deviation of the eyes.
Term
What are the other forms of a strabismus?
Definition
Esotropia: Eye turns medially (crossed eyes), Exotropia: Eye turns laterally (wall eyes), Hypertrophia: Eye turns upward.
Term
Why do we do adjustable suture surgery?
Definition
For adults with a diplopia stabismus, the suture technique allows a more accurate alignment and the field of vision is more exact.
Term
When can an adjustable suture surgery be performed?
Definition
After the anesthesia has worn off from their stabismus recession but not more than 24 hours after surgery.
Term
What are the two ways to correct a Stabismus?
Definition
Recession: recess muscles especially the vertically acting muscles.
Resection: Is when you remove a section of muscle. Greater affect on the lateral rectus.
Term
What instruments can be used to make the first incision? Where is the incision made?
Definition
A radial incision can be made with either westcott scissors or a disposable eye knife.
Term
Why is a second radial incision made?
Definition
To create a flap of conjunctiva Tenons capsule that can be dissected to expose the upper and lower border of muscle.
Term
What sutures are used to apply traction before the muscle is reattached at the new insertion point?
Definition
4-0 or 6-0 vicryl or silk sutures.
Term
Why do you use a strabismus hook?
Definition
To free the muscle from the sclera and move it forward.
Term
What type of sutures would be used to close the conjunctiva ?
Definition
6-0 absorbable or 7-0 chromic gut.
Term
When doing a recession by adjustable suture, what suture is used to secure it?
Definition
A double armed 6-0 polyglactin suture.
Term
What is the difference between closing the conjunctiva when doing a strabismus correction verses doing a recession by adjustable suture?
Definition
In the stabismus correction it is closed with an absorbable suture, with the recession it is closed using silk so that the slipknot is exposed for later adjustment.
Term
Why would you use a caliper for a strabismus correction?
Definition
To measure the distance from the original point of insertion to the new one.
Term
What is the function of the extrinsic eye muscles?
Definition
To move the eye in a coordinated manner, in various directions. And each muscle is associated with one primary movement but any eye movement can involve more than one muscle.
