Definition

Trade: fluticasone

Indications: seasonal and perennial allergies

Contraindications: Hypersensitivity 

Side Effects: itchy nose, runny nose, allergic reaction

Dosage and Route: Intranasal 2 prays each nostril once daily, not to exceed 2 sprays each nostril in 24 hr period

Other: anti-inflammatory (steroidal), corticosteroids

Definition

Trade: atrovent

Indications: maintenance therapy for airway obstruction due to COPD

Contraindications: Hypersensitivity to ipratropium, atropine, belladonna alkaloids, or bromide. Avoid using during acute bronchospasm Geriatric: may be more sensitive to effects

Side Effects: hypotension, palpitations

Dosage and Route: MDI 2 puffs QID, max 12 puffs in 24 hrs

Other: bronchodilators, anticholinergies

Definition

Trade: prinivil, zestril, qbrelis

Indications: hypertension management, HF management

Contraindications: concurrent use with aliskiren in patients with diabetes or moderate-severe renal impairment Geriatric: decreased initial dose recommended

Side Effects: hypotension, angioedema, dizziness, cough

Dosage and Route: PO 5-10mg once daily, may be titrated increase 20-40mg once daily

Other: any-hypertensives, ace inhibitors, monitor BP weekly, diabetics monitor BS frequently first month

Trade: cyclosporine ophthalmic

Indications: chronic dry eyes

Contraindications: hypersensitivity to cyclosporine

Side Effects: severe eye pain, burning, swelling, watery eyes

Dosage and Route: 1 drop per eye 2 times a day Q12 hrs

Other: ophthalmic anti-inflammatory agent

Definition

Trade: lurasidone

Indication: schizophrenia, depressive bipolar

Contraindication: concurrent use of CYP3A4 inhibitors or inducers Geriatric: increase risk of seizures, increase risk of death and stroke in elderly with dementia

Side Effects: agranulocytosis, seizures, neuroleptic malignant syndrome

Dosage and Route: PO 40mg once daiy, not to exceed 160mg in 24 hrs

Other: anti-psychotics, benzoisothiazole, BEERS List

Definition

Trade: latanoprost

Indications: glaucoma, ocular hypertension

Contraindications: hypersensitivity to lantanoprost benzalkonium chloride

Side Effects: eye discharge, redness, double vision, pink eye

Dosage and Route: 1 drop to affected eye once daily in evening, not to exceed 1 drop in 24 hrs

Other: anti-glaucoma, prostaglandin agonists

Definition

Trade: mometasone

Indications: nasal symptoms from seasonal and perennial allergies, nasal polyps

Contraindications: allergy to mometasone

Side Effects: headache, pharyngitis

Dosage and Route: Intranasal 2 sprays in each nostril once daily, not to exceed 2 sprays each nostril daily.

Other: anti-inflammatory, corticosteroids

Definition

Trade: carbatrol, mazepine, TEGretol, Epitol, Equetno, Teril

Indications: tonic-clonic, mixed, and complex partial seizures

Contraindications: allergy to carbamazepine or tricyclic antidepressants, bone marrow suppression

Side Effects: DRESS, SJS, toxic epidermal necrolysis, hepatotoxicity, pancreatitis, agranulocytosis, aplastic anemia, thrombocytopenia

Dosage an Route: PO 200mg daily, not to exceed 1600mg daily

Other: anticonvulsants, mood stabilizers, Grapefruit juice may increase effects; DNC extended-release tablets

Trade: fluconazole

Indications: fungal infections caused by susceptible organisms -UTI and Candida

Contraindications: pregnancy, concurrent use with pimozide, erythromycin, or quinidine Geriatric: increase risk adverse reactions

Side Effects: torsades, SJS, hepatotoxicity

Dosage and Route: PO IV 200mg initially, 100mg daily for 2 weeks

Other: antifungals (systematic)

Indications: treatment of vulvovaginal candidia

Contraindications: allergy to active ingredients

Side Effects: cramping, itching, vulvo burning

Dosage and Route: vag suppoasitroies 100mg at HS for 7 days, vag cream 1 applicatorful @HS for 7 days

Other: antifungals (vaginal)

Definition

Trade: methadose

Indications: moderate to chronic pain in opioid-tolerant patients

Contraindications: respiratory depression, acute or severe asthma, concurrent MAO inhibitor

Side Effects: torsades de pointes hypotension, constipation, respiratory depression, confusion

Dosage and Route: PO 2.5mg every 8-12 hr, PO 15-40mg once daily or dosage to prevent withdrawal

Other: opioid analgesics, opioid agonists, controlled substance II, weekly BP checks

Indications: overactive bladder

Contraindications: allergy to tolterodine or fesoterodine, urinary retention, gastric retention

Side Effects: SJS, dry mouth, headache, dizziness, severe allergic reaction

Dosage and Route: PO 2mg twice daily -imm. release; PO 4mg once daily - ext. release

Other: urinary tract antispasmodics, anticholinergic
