Diagnosis
Term
What is a life-size reproduction of a part or parts of the oral cavity and/or facial structures for the purpose of study and treatment planning
Definition
Diagnostic Cast (often called study cast)
Term
What is a negative likeness or copy in a reverse of the surface of an object
Definition
impression
Term
What is useful to duplicate the patient and the patient's jaw relations and movements
Definition
mounted diagnostic casts (MDC)
Term
What are the criteria for acceptability of the impression and casts and what does it entail?
Definition
  1. Accurate- is free of distortion
  2. Complete- has recorded all of the required anatomical parts
  3. Detailed- complete reproduction of surface detail

A complete cast is not always an accurate cast

Term
What is the proper technique with the patient to have an accurate diagnostic cast
Definition
  • Tray selection and modification
  • flexing of the mandible
  • removal without distortion
  • proper disinfection procedures
Term
What is the proper technique with the dental materials to make an accurate diagnostic cast
Definition
  • Proper storage of impression
  • measured amounts W/P Ratio when mixing both alginate and the dental stone
  • Proper mixing-Vacuum is preferred
  • Proper pouring technique - 2 stage pour
  • Proper setting-Removal-trimming
Term
What is a recepticle into which suitable impression material is placed to make a negative likeness
Definition
Impression tray
Term
What are the criterias for impression trays
Definition
  • Rigid- accuracy
  • Retain impression material
  • sterile-infection control
  • proper size-assortment
  • economical to use
Term
What are the types of impression trays and which one are we using?
Definition
  1. Stock tray
    • styrofoam-disposable
    • plastic-disposable(what we're using)
    • metal-perforated-reusable
    • metal-solid-reusable
  2. Individualized tray - modified stock tray
  3. Custom tray - made for a specific patient (out of acrylic)
Term
In the 3 different tray sizes what is alike and what is different?
Definition
the curve is the same, the anterior-posterior lengths are different
Term
How do you modify the tray?
Definition
Can heat flange (with Bunsen burner) and re-shape the plastic with fingers.  Cool the tray under running water
Term
What is the proper clearance between teeth and tray?
Definition
3-6 mm (1/4 in) both facially and lingually
Term
What is the type of wax we use to modify the tray and what is other item that can also be used
Definition

red rope wax

compound (requires heat to mold)

Term
What must be placed on the inside of the tray and wax and on the rim (edge) of tray?
Definition
Paint adhesive (TAC)
Term
What alginate also known as and what is it composed of
Definition

Irreversible Hydrocolloid

A hydrophilic elastic gel- a blend of dried sodium salt of Alginic Acid which is derived from a marine plant (12-18%) Calcium sulfate (12-15%) and water.

It contains sodium phosphate (2-3%) as a retarder and fillers such as diatomaceous earth and silicate powders (60-74%)

Term
How is alginate packaged
Definition
bulk and pre-weighed envelopes
Term
What is the water-powder ratio (W/P Ratio) for alginate
Definition

one scoop to one measure of water

Use 3 scoops for maxillary tray

2 for mandibular arch

Term
What is the timing for alginate
Definition

Mixing= 45-60 seconds

Loading the tray = 30 seconds

Setting time = 2 min 15 seconds

Impression should be poured in 10-15 min

Term
What does inadequate mixing result in?
Definition
a grainy mix and a poor recording of surface detail
Term
What are the mixing times for vacuum mix, regular set hand mix, and fast set hand mix
Definition
10 sec (only with stone), 45-60 sec (we use), 45 sec
Term
How do you seat a tray?
Definition
  • Proper patient and clinician position
  • Retract the lip with a mirror
  • rotate tray into position - use incisors as a guide
  • seat the tray - do not let the teeth touch the tray
  • seat time= 15-20 sec
Term
What is the operator position for lower impression
Definition
7:00-8:00
Term
What is the operator position for upper impression
Definition
11:00
Term
What are the steps to mixing the stone?
Definition
  • Use Microstone- prepackaged 140gm
  • measure 40 ml water
  • water in first then the powder
Term
What are the time increments to mixing the stone?
Definition

Hand mix (with flat spatula)= 10-15 sec

Vacuum mix= 20 sec

Working time = 6-8 min

Separation time (final set) = 45 min

Term
What is the name of the bowl used to vacuum mix?
Definition
Whip mix Vacu-spat bowl
Term
What is CEJ
Definition
Junction of crown and root
Term
What are the steps to pouring an impression
Definition
  • Impression should be moist
  • Add small increments from same area and vibrate until excess runs out the other side
  • fill the impression and add retention "knobs"
  • Set for 45 min
Term
What are the steps to checking the vacuum?
Definition
  • Vacuum = 0 pressure when machine is off
  • about 10 Hg when turned on
  • Cover nozzle. Gauge should be in the green zone (25-30) in Hg
Term
List the steps to recovering the cast
Definition
  • Remove any stone that would lock-on and prevent easy removal of the cast
  • Separate cast from the impression and rinse
  • Trim to desired form for mounting on the Articulator or
  • Add base to casts with second mix if a flat base is required
Term
What are equipment maintenance steps when making the cast
Definition
  • Run vacuum mixer for 20-30 sec after mixing to purge water from system and protect the engine
  • Do not get stone up into the vacuum hose- if you do, wash it out immediately
  • be sure to was out vacuum mixing bowl and paddle
Term
List the steps to cast trimming
Definition
  • Wet cast first
  • do not trim away teeth or anatomic parts that you need
  • wear safety glasses
  • run water through cast trimmer until effluent water is totally clear
Term
What are the cast detailing and storage steps
Definition
  • Allow casts to dry after trimming
  • remove all positive bubbles (blebs) carefully with a discoid-cleoid carver
  • Do not hand articulate the casts to see how they fit together (until dry-24 hrs later)
