An adverse drug effect that produces an unusual and unexpected response that is not related to the pharmacologic action of the drug.  Vary from patient to patient.

 

They are not classified as allergies b/c they are not immune related.

 

No one knows why, they just happen!

occurs when a substance increases or decreases a drug's actions.  The substance causing the drug interaction (must be external to the body) may be another drug, a dietary supplement, an herbal product, or a food.

Two drugs from a similar therapeutic class produce a combined summation response. 

 

Usually used for treating hypertension. 

ie. combination of a diuretic: used to lower SBP (10 mmHg) and a beta blocker added to produce another drop of 15 mmHG.  Combined drugs produced a drop of 25 mmHg

 

1 pain med + 2nd pain med = more effective

What is grapefruit juices effect on oral medication?

 

What drugs do they specifically act on?

Definition

Grapefruit juice inhibits the enzyme CYP3A4 in the wall of the intestinal tract. 

 

As drugs are absorbed they are NOT inactivated by CYP3A4, and higher amounts then reach the circulation. 

 

Inhibition may last up to 3 days after drinking the juice.

 

Substances in the juice are called furanocoumarins
