  • Segmental Systolic Blood Pressure Measurements (doppler)
Term
Therapeutic Procedures for PAD
Definition
  • Percutaneous Transluminal Angioplasty (PTA)
    • Balloon - used for poor surgical candidates or in cases where amputation is inevitable.
  • Laser-Assisted Angioplasty
  • Atherectomy - removal of plaque with rotary
  • Arterial Revascularization
  • Bypass Grafts
Term
Diagnostic Tests for PVD
Definition
  • Venous duplex ultrasonography
  • Venogram
  • MRI
  • D-Dimer test
Term
D-Dimer Test
Definition
  • Measures fibrin degradation products produced from fibrinolysis.
  • It can be very useful as an adjunct to noninvasive testing.
  • A negative D-dimer test can exclude a DVT without an ultrasound
Term
Therapeutic procedures for Varicose Veins
Definition
  • Sclerotherapy
    • Solution injected into vein, followed by pressure dressing
    • Incision and drainage of trapped blood 14 - 21 days after injection, followed by pressure dsg for 12-18hrs.
  • Vein Stripping
    • Veins are removed if they are larger than 4mm or in clusters
  • Radiofrequency Energy
    • Vein is heated from the inside by the RF and shrinks.
    • Collateral veins take over
  • Laser Treatment
    • Endovenous laser treatment uses a laser fiber to heat and close the main vessel that is contributing to the varicosity.
Term
Nursing Interventions for DVT and Thrombophlebitis
Definition
  • REST
    • Bedrest and elevation above heart.  AVOID knee gatch or pillow under knees.
    • Intermittent or continuous warm moist compresses
      • Prevents thrombus from dislodging and becoming an embolus, DO NOT massage
    • Provide thigh-high compression or anti-embolism stockings
  • Administer Medications
    • Anticoagulants
    • Thrombolytic Therapy
    • Analgesics
Term
Unfractionated Heparin (UFH)
Definition
  • Given IV
  • Given to preven formation of other clots
  • Prevent enlargement of existing clot
  • Followed up with PO anticoagulation with warfarin
  • Monitor aPTT, platelet counts
Term
Antidote for Heparin
Definition
  • Protamine Sulfate
Term
Warfarin
Definition
  • Works in the liver to inhibit synthesis of the four vitamin K-dependant clotting factors
  • Take 3-4 days before it can exert therapeutic anticoagulation.
  • Heparin is continued until Coumadin is therapeutic
  • Thearpeutic level are measured using INR.
  • INR is more accurate than PT, but some labs report both
Term
Antidote for Warfarin
Definition
Vitamin K
Term
When is thrombolytic therapy contraindictated?
Definition
  • Following surgery
  • Childbirth
  • Trauma
  • CVA
  • Spinal Injury
Term
Inferior Vena Cava Interruption Surgery
Definition
A filter traps emboli and prevents them from reaching the heart
Term
Nursing interventions for venous insufficiency
Definition
  • Elevate legs above heart for at least 20min 4-5x/day
  • Avoid prolonged sitting or standing, constrictive clothing, or crossing legs
  • Wear elastic or compression stockings
    • put on before getting out of bed after sleep
    • Clean each day, keep seems to outside
  • If prescribed, intermittent sequential pneumatic compression system
    • BID for 1 hr each time
    • Applied over open ulcer dressing
