Definition
The purpose is to focus on the human response to the disease state of pathophysiology. (fear, body image, disturbance, pain)

Definition
To gather info about the client that can be used in diagnosing, identifying outcomes, planning, and implementing care. Collect, validate, and organize data from patient and family.
