Term
Isoflurane
Definition
  • MOA: Depress RAS; SC dorsal horn neurons->sedation in frontal cortex->hypnosis on thal->SC motor neurons
  • USE: most common inhalational
  • TOX: no organ tox, safe for heart ds; decr peripheral res->decr bp, decr resp, decr cerebral vasc res-> incr CBF
  • PHARM: low blood solubility: 1.4, 99% excreted unchanged in lungs
Term
Nitrous Oxide
Definition
  • MOA: Blocks NMDA R (great analgesic)
  • USE: anesthetic in dentistry, analgesic, adjuvant to other anesthetics->allows decr in need for halogenated anesthetics->decr side effects
  • TOX: bronchodilation, decr mucociliary fx, decr metabolic rate, incr CBF, decr renal, liver BF, decr uterine contractions
  • PHARM: blood solubility 0.4 (fastest induction), fastest elim, 99% excreted unchanged in lungs
Term
Thiopental
Definition
  • Barbiturate, "truth drug", IV
  • MOA: lipophilic, rapidly enters & depresses CNS(<1min)
  • USE: induction of anesthesia(prevents excitement), NOT analgesic, good for pt with cerebral swelling
  • TOX: decr cerebral metab, CBF, CO, BP, resp depression
  • PHARM: short acting, diffuses fr brain to tissues
Term
Midazolam
Definition
  • Benzodiazepene, IV
  • USE: anxiolytic prior to anesthesia(15-60 min), sedation for non general anesthesia procedures
  • PHARM: water soluble, rapid onset and shorter duration than other benzos
Term
Fentanyl
Definition
  • MOA: Inhib presynaptic Ca->inhib release of pain nt
  • USE: induction and maintenace of IV anesthesia, epidural anesthesia (w/local anesthetics)
  • TOX: potent resp depression, chest wall rigidity, impaired ventilation, decr HR and bp
Term
Propofol
Definition
  • MOA: potentiates GABAa R
  • USE: Anesthesia Induction & Maintenance, day surgery(v. little vomiting), poor analgesic
  • TOX: bact contamination(formulated as an emulsion), resp depression, decr cardiac contractility, periph res, ICP
  • PHARM: onset as rapid as barbs but more rapid recovery
Term
Procaine
Definition
  • short acting
Term
Tetracaine
Definition
  • Long acting ester
  • MOA: Block volt gated Na channels from i/s when channel open, high firing freq fibers blocked first, decr a.p.->nerve conduction fails
  • USE: opthalmology, derm, plastics, ENT, nerve block, Bier block, epidural
  • TOX: restlessness, tremor, at high dose-convulsions, resp depression
  • PHARM: hydrolyzed by plasma esterases
Term
Lidocaine
Definition
  • Medium duration amide, (see Esters)
Term
Bupivicane
Definition
  • Long acting Amide anesthetic--See ester actions
