O - Spinous process of last six thoracic vertebrae, Last 3 or 4 ribs, Thoracolumbar aponeurosis and posterior iliac crest

I - Crest of lesser tubercle of the humerous 

Teres Major

A - Extend, Adduct, Medially rotate the shoulder

O - Lateral side of inferior angle and lower half of lateral border of the Scapula

I - Crest of the lesser tubercle of the humerous 

Deltoid

A - Abduct, Flex, Medially, Laterally rotate, Horizontally adduct and abduct, extend shoulder

O - Lateral 1/3 of clavical, Acromion, Spine of Scapula

I - Deltoid Tuberosity 

Trapezius
