IV drug user/abuser, large vegetation tricuspid valve so right sided disease, Staphylococcus aureus catalase positive, coagulase positive, gram positive cocci in clusters, new murmur, high fever, very sick, often normal valve

Subacute bacterial endocarditis
Definition

Viridans streptococci (S. mutans, S. sanguis) often cause of periodontitis, occurs on diseased/abnormal valve so history of rheumatic fever or congenital biscuspid aortic valve or calcific aortic sclerosis, no antibiotic prophylaxis before dental procedure (teeth cleaning, oral surgery, dental caries with filling), cystoscopy, endoscopy, lower grade fever, malaise, splenomegaly, Janeway lesions, Osler’s nodes, Roth spots, subungual splinter hemorrhages
