  • therapist: behind pt with hands on pts upper trap or top of head
  • tech: apply steady vertical pressure down through shoulder or crown of head
  • note: neg test-force is progressively transmitted through the body to feet or base of support
  • positive: breaking of psotural stability at any segment (ant shear in spine common)

Definition
  • use: provocation test of the posterior elements of the cervical spine
  • pt: sit in chair
  • therapist: at pt. side, help pt. move neck into  exn., side bending, and rotation to one side
  • tech: with pts neck in exn, side bending, and rotation to one side apply a vertical compression to head (slow and monitor)
  • perform first in uninvolved side, then do involved side
  • positive test: pain radiating into arm on side of neck is side bent and rotated to

Definition
  • provocation test of posterior elements of the cervical spine
  • pt: sitting in chair
  • standing at pts side, help pt move neck into exn, and rotation to one side
  • tech: pt's neck in exn and roation to one side apply parallel forces at maxilla and scapula while watching and listening to pt
  • note: pos test-reproduction of neurologic signs/symptoms, and increased intrascapular pain (cloward's sign)

Definition
  • determine subluxation of the atlas on the axis
  • pt: sitting
  • therapist: standing at side of pt. with one hand on pt. forehead and other hand over spinous process of axis to stabilize it
  • tech: pt asked to slowly flex the neck, while therapist resists neck flexion through forehead contact
  • note: positive test-if felt that head slides psoterior during movement

  • use: test vertebral artery function
  • pt: suprine on plith with head hanging over end of table
  • therapist: standing at head end of plinth, therapist supports head
  • tech: therapist takes pt head/neck into meximal exn, sidebending and rotation to same side, position is held for 30 seconds, while therapist observes nystagmus, dizziness, diaphoresis, nausea, pupillary changes
  • note: positive-would be indicated by any of the above-mentioned phenomena occurring

Definition
  • test for thoracic outlet syndrome
  • pt: standing with arms abducted to 90, ER at shoulder, and flexed to 90 at elbows
  • therapist: standing facing pt, observing
  • pt opens hands slowly for 3 min
  • pos test: pt being unable to hold arms overhead for assigned time, heaviness of the arms, weakness of the arms, numbness/tingling of the arm occurring during the test period

  • pt: standing or sitting
  • therapist: locate pt's radial pulse
  • tech: pts head rotated to face the test side, pt extends neck while therapist externally rotates ande xtends pt's shoulder. pt is then instructed to take a deep breath and hold it
  • note: disappearance of pusle-positive

  • pt: standing
  • therapist: palpates radial pulse while standing at side to be tested
  • technique: monitor pulse as pt's arm is drawn down and back
  • note: positive-absence of pulse
  • useful for pt's who report symptoms with wearing a backpack

  • stabilizing hand: therapist right hand cups upper cervical spine and occiput
  • mobilizing hand: therapist left index finger prox phalanx contact first rib
  • landmarks: posterior to clavicle, lateral to C7 transverse process, anterior to the spine of the scapula
  • tech: pt instructed to take a deep breath, therapist allow this to happen then follow rib downard as pt breathes out and then hold rib down, pt breathes normally, the patient breathes normally
