Definition
Last name, first name, middle name or middle initial. Name with only two units is filed before a name is three units, a unit with only an initial is filed before a full name beginning with that initial, business names are indexed in order of the names in the business, if last name has prefix (Mc, Mac, Van, de, Des or D) it is considered part of the last name therefor the first indexing unit, abbreviated first and last names be filed as written, nicknames be filed as if it were the legal name, hyphenated names are indexed as one unit, disregard titles unless the complete name is not given or unless they are necessary to distinguish between two individuals with exact same name, seniority terms such as Jr may be indexed as last unit, numeric seniority terms are indexed in numeric order before alphabetical terms, married women may retain her original name or use maiden and married name. If two last names are used they may or may not hyphenated, it may be necessary to cross index to prevent confusion, names of companies and businesses are indexed in the same order as written, disregard punctuation, articles such as a, an, the, acronym is indexed as one word, Identical names index them first under the name, and then under the location, beginning with the city as the first unit, state as second, street as third, and street number from lowest to highest
