Smell.

Pass strong smelling substance, observe for startle response.

Visual acuity-is checked with Snellen eye chart.

Visual fields-are checked with confrontation test

Fundalscopic exam-checks for pupil constriction and red light reflex.

Pupil condition-checks for accomodation and response to light.

• Extra ocular movements are tested with the cardinal fields of gaze (IV & VI).
• Cover/Uncover test (tropia)

• Extra ocular movement is tested by the six cardinal fields of gaze (III & VI).

Light touch- cotton whisp on bilateral face.

Pain/temperature-sharp object and cold tuning fork.

Corneal reflex-cotton whisp.

Mastication-chewing motion.

Inspect for facial asymmetry and involuntary movements.

1) Raise both eyebrows

2) Close both eyes tightly so that you can not open them. Test muscular strength by trying to open them

3) Puff out both cheeks

Hearing: Whisper numbers in one ear and ask  patient to repeat numbers. Make sure to cover the opposite ear.

Rinne test 

 Weber test

Gag reflex

Swallow

Movement of palate/uvula

Open mouth say "ahh"

Testing spinal accessory.

• Shrug shoulders
• Turn head from side to side

Inspect for tongue atrophy, fasciculations or asymmetry.
