Atrophic vag- pain, dryness, itching, dyspareunia, decreased rugae, dry, pale pink

- mood changes, depression, sleep disturbances, osteoporosis

Vulvovaginitis
Definition
Candida- White cheese-like→ pseudohyphae (KOH)→ Monistat
• Rx: Diflucan 100mg tab x 1, if abx recommend daily yogurt or lactobacillis

BV- fishlike vag. Odor= white-gray discharge→ clue cells (normal saline)+Whiff
• Rx: Flagyl 500 mg BID x 7 days or 2 grams

Trich- frothy, green discharge, stinky, pruritic, reddened vaginal area, dysuria, strawberry cervix→flagella -> trichomonads (Normal saline)
• Rx: Flagyl 2g PO x 1, treat partner or 500 mg bid x 7 days
**only one that is considered sexually transmitted

Menopausal vasomotor instability: hormonal therapy
Definition
- May use low dose combined hormone therapy; if contraindicated, may use low dose SSRI/SNRI and/or gabapentin
- estrogen (premarin), progestin (not necessary if hysterectomy)
- Topical estrogen may be used for atrophic vaginitis

DO NOT GIVE IF: fh of breast cancer, MI/CAD, uterine cancer

Definition
Urge Incontinence
o Due to detrusor overactivity (overactive bladder) leading to urgency, involuntary urinary loss, nocturia, and frequency

Stress Incontinence
o Due to pelvic floor and urethral muscle weakness leading to urine leakage from activities with increased pressure on bladder

Definition
o Physical exam findings: CMT, adnexal tenderness, abdominal tenderness, fever, dysuria, lower abdominal pain
o Tx: Rocephin 250 mg IM x1 + Doxycycline 100 mg BID x 14 days +/- Flagyl 500 mg BID x 14 days

Definition
- soft, fleshy, Painless keratinized growth around anus, vulvovaginal area, penis, urethra, or perineum

Tx: Podophyllin, trichloroacetic acid (TCA) or bichloroacetic acid (BCA)
refer: cryotherapy, laser therapy, electrocautery, or excision

Prevention: Gardasil or Cervarix

Definition
- urethritis and cervicitis, usually asymptomatic
- Females: dysuria, frequency, green discharge, labial pain and swelling, lower abdominal pain, fever, dysmenorrhea
- Males: dysuria, frequency, white/yellowgreen discharge, testicular pain, n/v

TX: rocephin 250 mg IM x1

Definition
o 2-3 mm painless vesicle, bubo, or non-indurated ulcer w/ or w/o regional adenopathy one month later
o Tx: Doxycycline 100 mg BID x 21 days

Definition
Treponema pallidum
- Primary: Painless chancre, indurated ulcer
- Secondary: Flu like symptoms, variable skin rash on palmar and plantar surfaces, lymphadenopathy
- Latent: Asymptomatic, but will be seropositive
- Tertiary: Leukoplakia, Aortic aneurysm, Cardiac insufficiency, Meningitis, Hemiparesis/Hemiplegia

Tx: Penicillin G 2.4 million units IM (primary, secondary, early under 1 yr)
- Penicillin G 2.4 million IM x 3 weeks (tertiary or latent)
- if allergic, doxy 100 mg BID, erythromycin 500 mg QID

Definition
- often asymptomatic
- females: dysuria, discharge, bleeding, dyspareunia
- males: dysuria, thick, cloudy discharge, testicular pain

Tx: Azithromycin 1 gram PO x 1 dose
- OR doxycycline 100 mg BID x 7 days

Definition
o Cramping pain occurring with menstruation
o Primary cause due to high levels of prostaglandin in adolescent women
• Self-limiting
• May be relieved with Ibuprofen, Oral contraceptive pills, Exercise, High fiber diet and Reduction of sugar/caffeine/salt
o Secondary cause associated with pelvic disease

o Menorrhagia (Regular frequency but prolonged bleeding)

o Menometrorrhagia (Irregular frequency and prolonged bleeding)
