Term
What is the MOA of Gentacicin?
Definition
Aminoglycoside
Mechanism of Action: Bind to 30S ribosomal subunit, which inhibits protein synthesis
Term
What are the adverse effects of Gentamicin?
Definition
Ototoxicity, Nephrotoxicity, neuromuscular blockade (treated with Calcium Gluconate IV), and Hypersensitivity, blood dyscrasias, streptomycin associated with neurologic disorders, neomycin causes suprainfections of bowel and intestinal malabsorption; topical neomycin can cause dermatitis
Term
What two antibiotics do you NOT want to mix IV?
Definition
Penicillin and Aminoglycosides
Term
Trough levels of Gentamicin correlates with what?
Definition
toxicity
Term
Peak levels of Gentamicin correlates with what?
Definition
Efficiay
Term
When giving Gentamicin its important to monitor what?
Definition
Renal function, serum levels, adverse effects Peak & trough/ BUN & creatine
Term
Gentamicin peak levels should be drawn when? (Daily / Divided) doses
Definition
Daily - don't have to
Divided - 1hr after IM , 30 min after IV
Should be b/n 4-10 mcg/ml
Term
Gentamicin trough levels should be drawn when? (Daily / Divided) doses
Definition
Daily - 1 hr prior to next dose
Divided - draw 15-30 min prior next dose
Should be 1-2 mcg/mL
Term
Tobramycin:
Definition
More active then gentamicin against Pseudomonas aeruginosa but less against enterococci and Serratia; nebulized tobramycin used for patients with cystic fibrosis
Term
What is Neomycin?
Definition
Most toxic aminoglycoside; not administered parenterally; used topically for eye, ear, and skin infections; orally to suppress bowel flora prior to surgery; can cause bowel suprainfection (eye drops for infection)
