What is a Lyphoma & what is the most common type?
Definition
A neoplastic transformation of cells in lymphatic tissue. 80% are B type. It is the most common neoplasm from 20-40yrs of age.

How do Lymphomas usually present?
Definition
  • May detect on yearly exam.
  • 2/3 have painless lymph adenopathy that persists 4-6w.
  • > 1cm NOT associated with infection
  • Usually upper body area.

  • Systemic complaints unusual but sometimes:
  • cough, chest discomfort, itching.
  • Abdominal fullness, chronic pain
  • Objective: abnormal CXR, thrombocytopenia

  • Tx based on type of tumor & less freqently by stage. Classified by pattern & rate of growth.
  • Low Grade: middle age/older, longer survival.
  • Intermediate Grade: short survival,
  • High Grade: aggressive, short survival.

What are some characteristics of a low grade Lymphoma presentation?
Definition
  • Generalized lymphadenopathy.
  • Normal CBC or mild lymphocytosis
  • Bone marrow involvement 75-90%
  • Can convert to more invasive
  • Survival 8-10y.

  • Large masses
  • frequent extranodial involvement.
  • Less frequent bone marrow involvement.
  • Diffuse large cell= neuro involvement & spreads rapidly.

  • Cancer only in one lymphnode area or organ outside the node.

  • Cancer is found in lymph node areas on done sode of the diaphram.
  • Found only in 1 area or organ outside the lymph node & in surrounding areas.
  • Other nodes on same side of the diaphram may also have cancer.

  • painless swelling of nodes, unilateral, usually neck, groin, axilla, abdomen.
  • Fatigue, itching, loss of appetite, recurrent fever, night sweats, neck pain, bone pain, wt loss.
  • 50% neck or supraclavicular area.
  • 70% superficial.
  • 60% mediastinal adenopathy
  • 25-30% night sweats, fever caled Pel-Epstein fever.
  • itching: maybe with rash.

  • Low parity is risk factor
