actions

Definition

*decr hr (chronotropic [rate])

*decr myocardial contractility (inotropic [force])

*decr rate of conduction thru AV node

Term

beta adrenergic blockers

cardioselective:metoprolol, atenolol

nonselective:propranolol, nadolol

therapeutic uses

Definition

angina pectoris

HTN

cardiac dysrhythmias

MI

HF

unlabeled uses include:

hyperthyroidism

migraine headache

stage fright

pheochromocytoma

glaucoma

Term

beta adrenergic blockers

cardioselective:metoprolol, atenolol

nonselective:propranolol, nadolol

side effects

Definition

bradycardia

dcr cardiac output

AV block

orthostatic hypotension

rebound myocardium excitation

bronchoconstriction

dcr glycogenolysis

Term

beta adrenergic blockers

cardioselective:metoprolol, atenolol

nonselective:propranolol, nadolol

contraindications/precautions

Definition

*beta adrenergic blockers are contraindicated in clients with AV block and sinus bradycardia

*nonselective beta adrenergic blockers are contraindicated in clients with asthma, bronchospasm and HF

*use cardioselective beta adrenergic blockers cautiously in clients with heart failure, asthma, bronchospasm, diabetes, hx of severe allergies and depression

Term

beta adrenergic blockers

cardioselective:metoprolol, atenolol

nonselective:propranolol, nadolol

medication/food interactions

Definition

*calcium channel blockers intensify the effects of beta blockers

*concurrent use of antihypertensive meds intensify hypotensive effects

*insulin prevents glycogenolysis

Term

beta adrenergic blockers

cardioselective:metoprolol, atenolol

nonselective:propranolol, nadolol

nursing interventions/client education

Definition

*do not discontinue med w/o consulting PCP

*avoid sudden changes in position to prevent orthostatic hypotension

*do not crush/chew ER tabs

*self monitor HR at home daily

Term

beta adrenergic blockers

cardioselective:metoprolol, atenolol

nonselective:propranolol, nadolol

nursing eval of med effectiveness

Definition

*absence of chest pain

*absence of dysrhythmias

*normotensive bp

*control of HF signs/symptoms
