Cards

Term
Colchicine
Definition

Clinical use: Treatment of acute attack, prophylaxis with chronic gout

MOA: disruption of urate deposition and subsequent inflammatory reaction

A/E: abdominal pain n/v/d

Term
Probenicid
Definition

Use: Gout prophylaxis, prevention of progression

MOA: decreases urate reabsorption in the proximal tubules.

-non-selective blockade of ACTIVE renaltransport of organic acid

-includes secretion and reabsorption

-effect depends on net effect of active secretion and reabsorption

Interactions: variety of drugs due to non-selective blockade, e.g. pcn naproxen

C/I: acute attack

A/E: hypersensitivity, gi sx

Teaching points: increase in freq duration and severity of attacks may occur the first 6-12 months of therapy

-to minimize stones maintain high fluid intake (2L+) and alkalinize the urine

Term
Allopurinol
Definition

MOA: inhibits uric acid production

Prodrug converted by xanthine oxidase

A/E: as uric acid decreases, dissolution of deposits can occur

--> disruption in urate equilibrium and acute attacks

*can give with colchicine
