Name For Adenosine
Definition
Adenosine (Adenocard)
Term
Class
Definition
  • Endogenous nucleoside(occurs naturally in all cells of the body)
Term
Action
Definition
  • slows electrical conduction through AV node, and interrupts re-entry pathway, convertingSVT to NSR
  • ONSET=within 30 seconds, Duration=1-2 Minutes
Term
INDICATION
Definition
  • SVT:Supraventricular Tachycardia

(s-127,s-163)

Term

Dosage/Route

(for SVT indication)

Definition

ADULT:

6mg rapid IV/IO; follow with 20ml NS IV/IO

12mg rapid IV/IO; follow with 20ml NS IV/IO

if no sinus pause MRx1 in 1-2"

If patienthas history of bronchospam or COPD:

Dosing as above per BHO

Peds:

PDC rapis IV:follow with 20ml NS BHO

PDC rapid IV: follow with 20ml BHPO

If no sinus pause MRx1 BHPO

Term
Contraindictions
Definition
  • 2nd and 3rd degree AV heart blocks
  • Sick sinus syndrome(without pacemaker)
Term
SIDE EFFECTS
Definition
  • SOB/dyspnea; may cause bronchospam in asthma patients (BHO)
  • Chest pressue/ palpations; may be acute in some patients for brief period.
  • Mild hypotension(decreases peripheral vascular resistance)
  • Dizzy/lightheadedness/ headache
  • Nausea
  • Transient arrhythmias(bradycardia, AV blocks, Ventricular ectopy) these are generally not treated and are quickly self-limiting.

SPECIAL INFORMATION: (CONCENTRATION: ?)

1-6

Definition

Concentration: 6mg/ 2ml

  1. DRUG IS METABOLIZED VERY QUICKLY, USE LARGE BORE iv IN LARGE PROXIMAL LARGE VEIN TO ASSIST ADMINISTRATION OF RAPID IV PUSH
  2. Use 20-ml syringe for NS flush immediately following (IV wide open is NOT effectice).
  3. Obtain ECG documentation before, during, and after giving adenosine.
  4. Transient arrhythnias (PVC's, PAC's, sinus bradycardia, AV block, sinus tach and possibly asystole) may be expected after each use.
  5. Do not give third dose if patient has  pause followed by return to SVT.  In this case, the patient is probably in A-Fib which will not be converted with Adenosine.
  6. Adenosine only works for SVT: will not convert A-FIB or A- Flutter
Term

Special Information

7-9

Definition
