Cervical Spine

Vertebral Artery Test

Definition
Slowly extend, rotate, and laterally flex the neck. Hold for 30 seconds

Test is positive if the pt. experiences dizziness, nystagmus (vibrating pupils) slurred speech, loss of consciousness.

End test/rx immediately - inform PT, physician or call 911

Foraminal Compression Test

Definition

Pt. is seated, clinician stand behind the pt, , pt laterally flexes neck. thenar eminance is placed on the superior aspect of the skull. Slight pressure is applied.

Positive is pain is reported in the UE toward the same side that the head is laterally flexed. May indicate nerve root compression, correlating to dermatomal distribution of pain.

 

Special considerations for a dx of RA, OA, OP, spinal stenosis.

Foraminal Distraction Test

 

Definition

Pt. is seated. Clinician places one hand under the chin the other around the occiput. Optional: Clinician is behind the pt. and places palms under the mastoid process of the temporal bone.
Distraction is in an upward motion.

Test is positive with reduction or elimination of pain (radicular).
Indicating nerve root compression.

Shoulder
Empty Can Test

Definition
Pt. is standing with shoulders abducted to 90 degrees, horizontally addicted 30 degrees and internally rotated so the subject's thumbs face the floor.

Clinician resists the pt. attempt to actively forward elevate both shoulders.

Positive involvement of the supraspinatus muscle and/or tendon is suspected with noted weakness or pain.

Speed's Test

 

Definition

Pt. is seated or standing. The involoved sholder is flexed to 90 degrees, the elbow is fully extended, and the foremarm is supinated. The clinician places one hand on the proximal aspect of the subjects hemeris near the are of the biciptal groove.

Clinician resists the subjects attempt to actively flex the humerous forward.

 

Positive for tenderness and/or painin the bicipital groove, possible bicep tendinitus.
