Olecranon Bursitis

Definition
Hx: Direct trauma or repetative grazing and weight bearing
Signs: pain, swelling, tenderness over olecranon, swelling, redness & heat.  Normal AROM & PROM, pain with overpressure, resistive testing normal
PT
Acute: RICE, splint, ionto (some evidence), education
Subacute: heat, ROM, sub-max iso's
Chronic: rehab residual problems, prevent recurrence, strengthening, functional training, protective padding

Bicepital Tendinitis

Definition
Hx:Repetative hyper/ of elbow with pronation or pronation-supination
Sx: pain at ant. distal humerus, pain with palpation as well
AROM: pain - shoulder & elbow √ and supination (lesser extent)
PROM: pain - shoulder & elbow / & pronation & overpressure
Resist: pain - elbow √ & supination
Special Tests: Yergason's, Mill's
PT: Modalities, transverse friction massage (>10min), muscle training, STM, education

Biceps Tendon Rupture

Definition
Hx: sudden cxn of biceps against large load, with elbow @ 90° √
Sx: sharp, tearing pain (acute) or swelling & activity related pain in cubital fossa (chronic), ecchymosis in cubital fossa, visible biceps defect (complete rupture)
AROM: pain - elbow √ & supination
PROM: pain - end range of / & pronation
Resist: weakness of elbow √, supination & grip
Med. manage: 1° repair - put tendon back, 2° repair - autologous graft, post op - protect 6-8 weeks
PT: modalities, ROM, gentle strengthening, education

Lateral Epicondylalgia

Definition
Hx: repetitive wrist /, rotation & grasping
Sx: gradual onset of pain, tenderness/swelling over lat. epicondyle, evening aching, night pain, morning stiffness, dropping things in pronated position
AROM: usually painfree full ROM, may be pain with wrist √ & elbow /
PROM: pain - wrist √, pronation with elbow /ed
Resist: pain - wrist /, radial deviation with elbow /ed, decreased grip strength, weak shoulder girdle and forearm
Special Tests: 
Resisted: wrist /, radial deviation, elbow /, 3rd finger /. Passive elbow / with wrist √.
Cozen's test. Mill's test
Rule out: Radial Tunnel Syndrome, C7 radiculopathy

 Lateral Epicondylalgia

(TREATMENT)

Definition
Med Manage: NSAIDs, corticosteroid inject. (contraindication for joint manip.), surgical debridement of common extensory tendon and/or release of the scarred area
PT:
Pain control: rest (splint), counterforce bracing modalities, STM, mob w/ movement
Exercise: stretching, proximal stabilization, plyometrics, functional training, total body conditioning, if cause is overuse - do not do repetitive tasks
Education: prevention techniques

Medial Epicondylitis (Golfer's Elbow)

Definition
Hx: repetitive √ & pronation
Sx: pain around medial epicondyle, flexor forearm --> wrist, tenderness over flexor-pronator origin
AROM: WFL, may have slight pain with wrist √
PROM: pain - wrist /, supination, elbow /
Resist: pain - wrist √ & pronation
Special Tests: Golfer's elbow test
Med. Manage: NSAID's, inject
PT: modalities, rest, ROM, stretch, strength, education
