What are Tags used for, and what is labeled on them.
Definition
The tag must identify the worker’s name, the worker’s employer, the date and time of lockout, the work
area involved, and the reason for the lockout.

Tags do not count as locks!

Types of Power Systems:
Definition
• Electrical—electrical panels, generators, lighting systems, etc.
• Mechanical (the energy of moving parts)—flywheels, blades, fans, conveyor belts, etc.
• Potential (stored energy that can be released during work)—suspended loads, compressed air, electrical capacitors, accumulated bulk goods, coiled springs, chemical reactions, changing states (solid—liquid—gas), etc.
• Hydraulic—presses, rams, cylinders, cranes, forklifts,etc.
• Pneumatic—lines, compression tanks, tools, etc.
• Thermal—steam, hot water, fire, etc.
• Chemical—flammable materials, corrosive substances, vapours, etc.
Term
Electrical Energy Systems: Identify
Definition
- control panels
- flexible wires
- which one controls the equip
-Trace the motor from the line to to the electrical disconnect
- Trace other systems connected to same power
Term
Electrical Energy Systems: Types
Definition
-Handle or Lever
- Double Throw Switch (up = normal, down = reversed, middle = stop)
- Rotarie Switch
Term
Is the operation control switch a valid location to produce a zero state?
Definition
NO! Machine can still be powered through backup systems or remotely.
Term
Steps to locking out a Electrical Energy Systems
Definition
-Stop the machine
-let it complete its Cycle
-Test the machine to make sure it is off.
-Lock out each energy source connecting to the equipment
Term
Pneumatic Energy System: Identify
Definition
-Piped inline
-air filter
-regulator
-lubricator
Term
What is a power valve?
Definition
It is a valve that directs air from a source to the various tools.
Term
Surge tanks are:
Definition
are tanks meant to retain air to activate a machine if the generator is off.
Term
What piece of equipment is found on a pneumatic system but not others?
Definition
Silencer
Term
Type of Safety lockout valves:
Definition
Piston valve out = on (in = off)
Lever style valve (not always lockable)
Tea Handle (valve out = off)
Term
Pneumatic Tool lock out procedure:
Definition
Trace System
Stop all Equip
Turn off lock out
Block area
Drain all stored air
Look for multiple feed systems
Test System
Term
Hydraulic Energy Systems: Identify
Definition
-reservoir
-pump
-electrical motor
- inline filter for oil
- one way check valves
-flow controls, regulates the speed of equip
- pressure reducing system
- hydraulic cylinders (linear movement)
Term
Hydraulic lock out procedure:
Definition
-stop the machine
-lock out the electrical system / pump motor.
-push button on side of hydraulic cylanoid valve to push oil to reservoir
-Test the System
Term
Special Energy Systems: Features
Definition
used for heating cooling
gas,steam,water,chemichals,or other liquids
Term
Special Energy Systems: Identify
Definition
labels
tags on valves
color coding
Term
What is special about locking out a gate valve?
Definition
gate valves are not equipped for lock out, you use chain and lock it in place.
Term
What can you do to make a physichal lock out to a piping system?
Definition
Mis align the pipes if needed.
Term
Gravity momentum stored energy
Definition
-Block it out, connect to the control panel with a lockout
-Block unit before releasing hydraulic pressure
-Block, pin, or clamp the chain.
-Wait for momentum to stop. Band saws.
-Springs, release the tention, on the bracket
Term
When in doubt?
Definition
4 rungs :P (yaya lock it out)
Term
Step 1:
Definition
Locate area. Identify Equipment, Machinery etc...

Identify the area with references such as floor, room name, elevation, or column number. Identify the equipment that is the subject of the work.
Term
Step 2:
Definition
Identify All Energy Sources

Identify all energy sources affecting the equipment or machinery. Identify the various energy forms to be locked
out such as electrical, momentum, pneumatic, hydraulic,
steam, and gravity.
Term
Step 3:
Definition
IDENTIFY THE PARTS TO BE LOCKED OUT
OR ISOLATED

Identify systems that affect, or are affected by, the work
being performed. These may include primary, secondary,
backup, or emergency systems and interlocked remote
equipment.
Review the current system drawings for remote energy
sources and, where required, identify and confirm with the
client or owner the existence and location of any switches,
power sources, controls, interlocks, or other devices
necessary to isolate the system.
Remember that equipment may also be affected by
• time restrictions for completing the work
• time-activated devices.
Term
Step 4:
Definition
DETERMINE LOCKOUT METHODS

Confirm that the lockout of all energy sources is possible.
Term
STEP 5:
Definition
NOTIFY ALL PERSONNEL AFFECTED
Term
Step 6:
Definition
SHUT DOWN EQUIPMENT AND MACHINERY

Trace all systems to locate and lock
out energy sources
Term
Step 7:
Definition
INSTALL LOCKOUT DEVICES

Merely removing a fuse
doesn’t constitute lockout.
The fuse could be easily
replaced. The fuse should
be removed and the box
locked out.
Term
Step 8:
Definition
TAGGING
Term
Step 9:
Definition
VERIFY ZERO-ENERGY STATE

verify that all personnel are clear of the equipment. Then try, with extreme caution, to start the equipment manually

Test the system to ensure that all electrical components are de-energized and de-activated, including interlocking and dependent systems that could feed into the system, either mechanically or electrically.

YES OR NO
No = hazardous energy not controlled Proceed to Step 2
Yes = Step 10
Term
Step 10:
Definition
Perform the Work
Term
Step 11:
Definition
COMMUNICATE THAT WORK IS COMPLETE
AND THAT ALL PERSONNEL ARE CLEAR

• Ensure that personnel are clear of the locked-out equipment, machinery, or system.
• Remove only your tags and locks.
• Tell personnel that were originally informed of the lockout that the equipment, machinery, or system is no longer locked out.
Term
Step 12:
Definition
RESTORE POWER

Return systems to operational status and the switches to power ON. Have qualified personnel restart machinery or equipment.

WORK STILL REQUIRED?
Yes = Step 6
No = Step 13
Term
Step 13:
Definition
RETURN CONTROL TO OPERATING
PERSONNEL

the person in charge of the
lockout operation should formally return control of the equipment or system to plant personnel.
Term
STEP 14:
Definition
RECORD DATE/TIME LOCKOUT REMOVED AND SYSTEM RESTORED
Term
Lockout and tagging procedures help to ensure that:
Definition
• all energy sources are identified and locked out
• energy is not inadvertently restored while work is proceeding
• maintenance, repair, installation, and other jobs can be carried out safely
• records are kept.
Term
Lockout is a procedure that:
Definition
prevents the release of hazardous energy. It often involves workers using a padlock to keep a switch in the “off” position, or to isolate the energy of moving parts
Term
A written procedure helps to ensure that lockout and
tagging have been thoroughly and effectively carried out before work begins. It should include:
Definition
• training requirements for workers and supervisors
• quality, type, and colour of locks, scissors, chains, blanks, blinds, and other lockout devices
• method of identifying lock owners
• control of keys for locks
• colour, shape, size, and material for tags
• method of securing tags and information to be included
• communication and authorization procedure for shutting down and starting up machinery and equipment
• record-keeping requirements
• itemized steps to meet lockout objectives.
Term
guards, locks, temporary ground cables, chains, tags, and other safeguards must not be tampered with or removed until:
Definition
a) the work is complete, and
b) each worker has removed his or her personal lock.
