Indications: hypothyroidism, pituitary TSH surpression

Contraindications: acute MI, angina pectoris of elderly, cortical insufficiency

Adverse Effects: weight loss [not for use as this in euthyroid pt]

Definition
Class: thyroid drug, goitrogen

Mechanism: inhibit thyroid peroxidase, inhibits deiodinase; prevents synthesis of thyroid hormone and conversion of T4 to T3

Indications: Graves', hyperthyroidism, toxic multinodular goiter, acutely prior to thyroidectomy or radioactive iodine; PREFERRED IN PREGNANT FEMALES

Contraindications: allergy

Adverse effects: severe liver injury and acute liver failure, inc. risk of bleeding w/ oral anticoags (dec. prothrombin)

Class: thyroid drugs, goitrogen

Mechanism: inhibit thyroid perxodiase; inhibits synthesis of thyroid hormones

Indications: Graves', hyperthyroidism, toxic multinodular goiter, acute tx before thyroidectomy or radioactive iodine; PREFERRED FOR HYPERTHYROIDISM (LESS TOXIC, DAILY DOSE)

Contraindications: pregnancy (crosses placenta and breast milk, assoc. w/ malformations)

Adverse effects: inc. risk of bleeding w/ oral anticoags (dec. prothrombin)

Class: thyroid drug

Mechanism: uptake and subsequent destruction of follicular cells by radiation (B particles and g-rays)

Indications: Graves', overactive/enlarged thyroid, thyroid cancer (thyroid ablation and metastatic disease)

Contraindications: pregnancy

Adverse effects: hypothyroidism (life-long replacement), thyroiditis, worsening ophthalmopathy in Graves'

Definition
Class: thyroid drugs

Mechanism: inhibtion of iodide uptake, inhibit iodination and organification, inhibit thyroid hormone release

Indications: emergency tx of thyroid storm, thyroid protection during tx (reduce size and vascularity before surgery), thyroid protect from nuclear accidents and emergencies

Contraindications: allergy

Adverse effects: hypothyroidism, may worsen toxic goiter symptoms

Pharmacokinetics:
Absorption - oral, reduced to iodide in intestine then absorbed; or IV
Distribution: rapid, decrease TH in 24hrs, but reversible and transient

Mechanism: inhibits 5'-deiodinase causing decrease T4/T3 conversion and increased conc. of rT3

Indication: hyperthyroidism symptoms, thyroid storm
