• Age = birth-18 months
• Identifies infants and toddlers with gross motor delay and evalutates gross motor skill maturation

• Age = 4-21 years
• Common assessment tool for higher level motor skills and to evaluate motor training programs

DDST-II

Denver Developmental Screening Test

• Age = 1 week-6.5 years
• Commonly used screening test to determine developmental delay, although the specificity is weak and the sample population may be biased

FPS-R

Faces Pain Scale

• Age = 4-16 years
• Measures self-reporting of pain intensity

GMFM

(Gross Motor Function Measure)

• Age = 5 months-16 years 
• Common assessment to determine quantity of movement in children with CP or Down Syndrome

M-ABC-2

Movement Assessment Battery for Children

• Age = 3-16 years
• Identifies impairments in motor function of children with milder movement disorders. Includes qualitative and quantitative information

PDMS-2

Peabody Development Motor Scales

• Age = 1-72 months
• Motor quotients are determined to estimate overall motor abilities. Commonly used in EI programs to determine eligibility for services. Has accompanying Motor Activities Program to assist in teaching skills

• Age = 5-15 years with mild to moderate motor impairments
• Used for screening or evaluation of functional balance skills

PEDI

Pediatric Evaluation of Disability Inventory

• Age = 6 months-7.5 years and those whose functional abilities are lower than those of a 7 y.o.
• Identifies the functional and activity capabilities of infants and children with disabilities. Used to monitor change in activities and functional skills and evaluate program outcomes

SFA

School Function Assessment

• Age = Children with disabilities in grades K-6
• Comprehensive assessment of activity and functional capabilites of children with disabilities in school setting. While not tied to a curriculum, information obtained assists in determining eligibility for service in development of educationally relevant objectives

TUG

Timed Up and Go

• Age = 3 years-adult, excluding those with cognitive impairment or unable to follow directions

• Age = 6 months- 8 years
• Determines degree of a child's disability, ability to accomplish tasks, and caregiver assistance required. Must use their data collection and outcome reporting system
