Pain

Definition
1) pain is whatever the experiencing person says it is, existing wherever he says it does (clinical)

2) pain is an unpleasant sensory and emotional experience associated with actual or potential tissue damage, or described in terms of such damage (physiologic)

3) nociception is the sensation associated with tissue damage, and it is the sensation that leads to the perception of pain (measurable outcome)
Term

physiology of pain

Definition
-transduction: nociceptors-action potential-converting to nervous system signal
-transmission (conduction): nerve fibers-conduction of action potential along nerve fibers through spinal cord to brain
-perception/interpretation: thalamus/cortex-expression/response
-modulation: neurotransmitters, etc
Term

conduction of nociceptive impulses:

classification of nerve fibers

Definition
-group I (A alpha fibers) fast impulse-have a lot of myelin- work in changing muscle fiber length, allow us to quickly change the rate of muscle contraction
-groupII (A, B fibers) fast impulse- have a lot of myelin- vibratory sense is detected and deep tissue touch detected
-group III (A delta fibers) nociception-fast pain- picking up acute pain-thinner myelin
-group IV (C fibers) nociception-slow pain- unmyelinated- burning, warm sensation of pain
Term

nociceptive pathways

Definition
stimulus-peripheral
spinothalamic tract-spinal cord
thalamus-pain perception
cortex-pain interpretation
Term

theories of pain

Definition
-specificity theory- amount of tissue injury is directly related to the intensity of pain

-pattern theory- different parts of your body gives off different signals and have different patterns when pain arises

-gate-control theory- different nerve fibers are putting are putting input when noxious stimuli is picked up the larger fibers info is going faster and will reach brain before info traveling on slow fibers. you have potential to override slow fibers
Term

classification of pain

Definition
by duration of symptoms:
-acute pain
-chronic pain

by location and referral pattern
-cutaneous and deep somatic pain
-visceral pain
-referred pain

by source:
-surgery, trauma, cancer, etc
Term

other types of pain

Definition
neuropathic pain-nerve damage (affects processing of pain)
-altered nervous system function
-thalamic syndrome, phantom limb pain, trigeminal neuralgia, diabetic neuropathy, postherpetic neuralgia

psychogenic pain
-no known physical cause
-pain disorder
