-          Morphine sulfate

Definition

o   All drugs compared to this

o   Roxanol- oral solution

§  Gallbladder disease

o   Very high hepatic first pass effect

o   Anti-tussive- main use to treat cough

o   Less effect than morphine to treat pain

Definition

o   Very potent

o   Much more powerful than morphine

o   1mg of this equals 10mg of morphine

o   Very short acting

o   PO, IV, and rectal sepository

-          Meperidine HCL (Demerol)

Definition

o   75mg of this equal 10mg of morphine

o   50% hepatic first pass effect

o   Whole lot orally to achieve

o   Very long half life

o   Can start to accumulate, cause CNS toxicities, within 48 hours of giving it consistently

o   Halucinations

o   Cannot have if they are taking or have taken within 3-4 weeks  MAOI

o   Very high BP, very high fevers, seizures, could cause death

-          Methadone(dolophine)

Definition

o   Drug choice for opiate detox

o   Used for patients with chronic pain

o   Used for babies that are addicted

-          Fentanyl

Definition

o   Very potent, much more potent than morphine

o   Used for acute, chronic pain

o   Transdermal, injections, lollipop, IV, IM

§  Patch is good for 3 days

§  Doesn’t start to work for 6-12 hours

§  Breakthrough pain

§  Lollipop used for patients with cancer

o   0.1 mg = 10 mg morphine IV, VERY POTENT!

-          Oxycodone (Roxicodone)

Definition

o   Oxy + Tylenol = Percocet, tylox, roxicet

o   Oxy + ASA = Percodan

o   0xyIR = immediate release

o   oxyCotin = timed release

o   hydrocodone + acetaminophen = vicodin, lorcet, lortab

o   hydrocodone + ibuprofen = vicoprofen

o   ask for meds BEFORE pain to severe

o   constipation intervention

§  drink lots of water, lots of fiber, 25 g of fiber

§  use foods instead of another medications

§  Competes with the other drug

§  Push a little and wait to see what happens

§  Long half life

§  Look for rebound pain

§  Fixed schedule for cancer patients not PRN
