Term
Ductus Arteriosus
Definition
Connects the pulmonary artery with the descending aorta; Closure occurs when the pulmonary resistance becomes less than system vascular resistance which leads to a left to right shunt leading to closure in about 15 hours after birth. May stay open is lungs do not expand or PaO2 levels drop.
Term
Ductus Venosus
Definition
Connects the umbilical vein to the inferior vena cava
Closes by day 3 of life and becomes a ligament; closure forces perfusion of the liver.
Term
2 arteries and 1 vein (if you have only 1 artery and 1 vein you need to look at the function of the nb's kidneys)
Definition
The umbilical cord has how many arteries and how many veins?
Term
Highest immediately after birth and lowest at about 3 hours of age
Normal findings: 50-75/30-45 mm Hg
Not typically taken; measure in both arms and both legs
Blood pressure varies during the first 12 hours of life according to birth weight (if blood pressure on legs is much lower than that is the sign of a heart defect; you would check B/P in all four areas if femoral pulse is not felt)
Definition
Blood pressure in a newborn
Term
convection (type of heat loss in nb)
Definition
heat is lost to air or fluid around the infant that is cooler than infant’s temperature
Term
radiation (heat loss in nb)
Definition
heat is lost to solid objects near the infant that are cooler than infant’s temperature
Term
conduction (heat loss in nb)
Definition
heat is lost to cold surfaces that come in contact with the infant
Term
evaporation (heat loss in nb)
Definition
heat is lost when water evaporates from the infant’s skin surface or respiratory tract (most susceptible at the time of birth)
Term
At about 40 mg/dL (transient from 48-72 hours of life)
Definition
when is a baby considered to have hypoglycemia?
Term
2500 to 4000 grams (5lb, 8 oz to 8lb 13 oz)
Definition
In a healthy nb the birth weight ranges from...
Term
33 to 35.5 cm (13 to 14 inches)
Definition
In a healthy nb the head circumference ranges from...
Term
30.5 to 33 cm (12 to 13 inches)
Definition
In a healthy nb the chest circumference ranges from...
Term
48 to 53 cm (19 to 21 inches)
Definition
In a healthy nb the length (from head to heel) ranges from...
Term
second period of reactivity
Definition
Follows the period of relative inactivity and lasts 2-8 hours
Varies between active alert and quiet alert state
Periods of rapid RR in response to stimuli and activity
HR varies in response to stimuli and activity
Increase bowel activity- may pass meconium stool
Neonate responds to external stimuli

Neonate responds to external stimuli
Term
period of relative inactivity
Definition
Begins at 30 minutes of age and lasts 2 hours
Sleep state
Neonate is unresponsive to external stimuli
RR decreases and can fall slightly below normal range
HR decreases and is within normal range
Oral mucus production decreases
Term
initial period of reactivity
Definition
1st 15-20 minutes post birth
Alert and active
Vigorously responds to external stimuli
RR irregular and may be rapid (can be as high as 90 bpm)
May exhibit momentary grunting, flaring and retractions.
Brief periods of apnea may occur
HR is rapid and can be as high as 180 bpm
Brief periods of cyanosis can occur
The amount of oral mucus increases
Term
SGA; small for gestational age
Definition
(<5lb, 8oz for a term infant); less growth occurred in utero than expected; associated risks include: hypoglycemia, asphyxia, RDS, meconium aspiration, intrauterine infection & hyperbilirubinemia [below the 10th percentile?]
Term
LGA (large for gestational age)
Definition
(>8lb, 13 oz for a term infant); associated risks include: birth trauma, hypoglycemia, hypocalcemia, meconium aspiration, polycythemia [between the 10th and 90th percentile]
Term
FIRST: make sure they are breathing
APGAR score
Complete head-to-toe assessment
Ophthalmic Erythromycin *
Umbilical cord care- some sites may use triple dye
Vitamin K (Aqua Mephyton) administration*
Blood glucose determination (if indicated)
Early breastfeeding
Definition
Immediate newborn cares
Term
Screening for inborn errors (PKU [amino acid] test, ppl who have PKU disease cannot breakdown this amino acid.. May cause mental retardation ect)
State Mandated- different states have different requirements
Done once infant has been feeding at least 24 hours

Hepatitis B Vaccination is offered

Definition
(nosocomial infection)
Transmitted from equipment or staff
Term
When levels are >6-7 mg/dl
Definition
At levels of bilirubin will a nb show jaundice?
Term
direct (conjugated) type of bilirubinemia
Definition
water soluble and easier for the body to eliminate
Term
indirect (unconjugated) type of bilirubin
