Definition

Precautions: Don’t take if allergic to morphine, asthma, GI obstruction, paralytic ileus, hepatic/renal impairment, severe respiratory depression, Interacts with alcohol and CNS depressants, MAOIs

Trade name: Pitocin, Syntocinon

Action: -Activates receptors that trigger increase in intracellular calcium levels

-Increase prostaglandin production

-Stimulates uterine contraction

Dosage: IV 10 units/mL, 30 units/mL

Intrapartal dose:

0.5-1 milliunit/min

Postpartum dose:

10-40 units in 1000 mL IV

10 units total after delivery

Route: IV, IM

Side Effects: Tachycardia, premature ventricular contraction, hypotension, N/V

Rare: Lacrimation, nasal irritation, rhinnorrhea, uterine bleeding/c contraction

Adverse: Hypertonicity

Precautions: Used only for medical reason, Do not use with caudal block anesthetics, vasopressors

Purpose:Induction of labor, Control postpartum hemmorhage

OB Purpose: Contracts uterus during birthing, Milk production

(Methergine)

 

Definition

Action: Increases tone, rate, amplitude of contraction of uterine smooth muscle

Dosage: Injection 0.2 mg/L Tablet 0.2 mg

Side Effects: N/V, uterine cramping, abdominal pain, diarrhea, dizzy, diaphoresis, tinnitus, bradycardia, chest pain ADVERSE: Hypertensive episode result in CVA, arrhythmias, seizures

Precautions: -Hypersensitivity, hypertension, pregnancy, toxemia

Caution: renal/ hepatic impairment, sepsis, protease inhibitors, cardiac issues

Purpose: Shortens third stage of labor, Reduces blood loss and possible complications

OB Purpose: Management of uterine atony, hemmorhage, and subinvolution of uterus after placenta delivery

Misoprostol (cytotec)

 

Definition

Action: Replaces protective prostaglandins consumed with prostaglandin- inhibiting therapies (NSAIDs). Induces uterine contractions

Side Effects: Abdominal pain, diarrhea, nausea, flatulence, dyspepsia, headache, vomit, constipation ADVERSE: Overdose

Precautions: Category X pregnancy, Renal impairment, cardiovascular disease, elderly, Antacids increase concentration

Purpose: Prevention of NSAID-induced gastric ulcers, medical termination of pregnancy 49 days or less

-Cervical ripening, labor induction, postpartum hemmorhage, treat incomplete abortion

Action: Relaxes smooth muscle at the cervix

Precautions:Cardiac, hepatic, pulmonary, renal disease

Purpose: Promote cervical ripening in pregnant women at or near term to induce, Stimulates myometrial contractions in gravid uterus

Definition

Action: Inhibits calcium ion  movement across cell membranes, depressing contraction of cardiac, vascular smooth muscle

Side Effects: Peripheral edema, headache, flushed skin, dizziness, nausea, shaking, muscle cramps pain, drowsy, palpitations, nasal congestion, cough, dyspnea, wheezing

RARE: Hypotension, rash, pruritic, urticaria, constipation, abdominal discomfort, flatulence, sexual dysfunction

Precautions: Strong CYP3A4 inducers. Do not eat with grapefruit

Purpose: Increase HR, myocardial o2 delivery, cardiac output

OB Purpose: Relax uterine contractions and postpone pre-term birth

Action: Blocks aplha1, beta1, beta2 adrenergic receptor sites

Side Effects: Drowsiness, dizziness, excessive fatigue. Occasional: dyspnea, peripheral edema, depression, anxiety, constipation, diarrhea, nasal congestion, weakness, diminished sex function, transient scalp tingling, insomnia, N/V, abdominal discomfort

Precautions: Renal functioning, monitor for hypotension

Purpose: Slows sinus heart rate, decreases peripheral cascular resistance, BP. Management of hypertension

Side Effects: Chalky taste, constipation, headache, dry mouth, irritability, decreased appetite, metallic taste, fatigue, weakness, depression

Precautions: Renal functioning, hypokalemia, concurrent use with digoxin

Precautions: Hypersensitivity, kidney problems

Purpose: Treat bacterial infections

OB Purpose: Safe during pregnancy, used to treat bacterial infections

Action: Inhibits cell wall synthesis in susceptible microorganisms

Side Effects: Systemic: Abdominal pain, nausea, vomiting, diarrhea. Topical: Dry, scaly skin. Vaginal: Vaginitis, pruritus. Occasional: Systemic: Phlebitis; pain, induration at IM injection site, allergic reaction, urticaria, pruritus. Topical: Contact dermatitis, abdominal pain, mild diarrhea, burning, stinging. Vaginal: Headach, dizziness, nausea, vomiting, abdominal pain. Rare: Vaginal: Hypersensitivity reaction.

Definition

Action: Man-made form of Vitamin K that allows blood to clot

Dosage: 41.5 and 52 mL/kg for the 0.2% and 1.0% concentrations

Route: SubQ Injection

Side Effects: Change in sense of taste, warmth or redness in face, neck, upper chest, arms

Precautions: Pregnant or breastfeeding, blood clots, liver disease, hx of stroke or embolism, allergy to vitamin K, drug interactions

Purpose: Helps blood clot

OB Purpose: Not for pregnant women or those who are breastfeeding

Definition

Action: Introduced topically into eye, to prevent eye infections in newborns

Dosage: 30-50 mg/day

Route: Topically

Side Effects: Stinging of eye, burning, eye irritation, redness

Precautions: Contact lenses weigh at least 15 mins before put back in, Other eye meds

Purpose: To prevent eye diseases like conjunctivitis

OB Purpose: To prevent eye diseases in newborns

 

 

Term

 Gentamicin

 

Definition

Action: Irreversibly binds to protein of bacterial ribosome.

Dosage: IM, IV: adults, elderly 1-2.5 mg/kg/ dose q8-12h. (once daily): 4-7 mg/kg/dose q24h. Children 5 years and older: 2-2.5 mg/kg/dose q8h. Infants, children younger than 5 years: 2.5 mg/kg/dose q8h. Neonates greater than 2 kg PNA 8-28 days or less: 4 mg/kg/dose q24h. Infants (1-2 kg): PNA 8-28 days: 4-5 mg/kg/dose q24h-q48h; PNA 7 days or less: 5 mg/kg/dose q48h. Infants less than 1-2 kg: PNA 15-28 days 4-5 mg/kg/dose q24-48h; PNA 14 days or less: 5 mg/kg/dose q48h.

Ophthalmic: ointment: adults, elderly ½ - inch strip to conjunctival sac 2-4 times/day. Solution: adults, elderly, children 1-2 drops q2-4h up to 2 drops/her. 

Route: IM, IV, ophthalmic

Side Effects: IM: pain, induration at injection site. IV: phlebitis, thrombophlebitis, hypersensitivity reactions (fever, pruritus, rash, urticaria). Ophthalmic: burning, tearing, itching, blurred vision.

Precations: Elderly, neonates due to renal insufficiency or immaturity, neuromuscular disorders(potential for respiratory suppression), vestibular or cochlear impairment, renal impairment, hypocalcemia, myasthenia gravis. Pediatric patients on extracorporeal membrane oxygenation 

Purpose: Treatment of infections susceptible to pseudomonas, proteus serratia, and other gram-negative organisms and gram-positive staphylococcus including skin/skin structure, bone, joint, respiratory tract, intra-abdominal, complicated urinary tract, acute pelvic infections; burns; septicemia; meningitis.

OB Purpose: Bacterial infections for mothers and newborns.

 

Term

Ampicillin

 

Definition

Action: Inhibits cell wall synthesis in susceptible microorganisms

 Dosage: PO: Adults, elderly: 250-500 mg q6h. Children: 50-100 mg/kg/day in divided doses q6h. Maximum: 2-4 g/day. IV, IM: Adults, elderly: 1-2 g q4-6h or 50-250 mg/kg/day in divided doses. Maximum: 12g/day. Children: 25-200 mg/kg/day in divided doses q6h. Maximum: 12g/day. Neonates: 50 mg/kg/dose q6-12h.

Route: PO, IV, and IM

Side Effects: Pain at IM injection site, GI disturbances (mild diarrhea, nausea, vomiting), oral or vaginal candidiasis.

Precations Contradictions: hypersensitivity to ampicillin or any penicillin. Infections cause by penicillinase-producing organisms. Cautions: history of allergies, esp. Cephalosporins, renal impairment, asthmatic patients, infectious mononucleosis.

Purpose: Treatment of susceptible infections to to streptococci, S. pneumoniae, staphylococci, meningococci, listeria, some klebsiella, e. Coli, H. influenzae, salmonella, shigella including GI, GU, respiratory infections, meningitis, endocarditis prophulaxis.

OB Purpose: To prevent neonatal infection during birth

Hepatitis B Vaccine

 

Definition

Action:Inactive form of the virus used to protect against it

Dosage: 2 or 3 doses depending on age: 5mcg, 10mcg, or 40 mcg

Route: IM

Side Effects: Soreness at site, slight fever

Precations: Severe life-threatening allergies, not feeling well at the time

Purpose: prevent hepatitis B and its consequences, including liver cancer and cirrhosis

OB Purpose: Given to baby within 24 hours after birth, followed by 2 more later

Definition

Action: Glycerin protects skin and a provides a protective barrier over hemorrhoids

Dosage: 6 times per day or after each BM

Route: Cream, pads

Side Effects: Bloody diarrhea, severe pain, bleeding, irritation around rectum. AE: alleric rxn.

Precuations: Avoid if allergic too

Purpose: Relieve rectal pain, pressure, irritation, and mild bleeding.

OB Purpose: Shrinks swollen tissue to provide relief from itching and irriatation

Definition

Action: Blocks nerve signals in body for local anesthetic

Dosage: NDC 53014-376-16 28 g tube NDC 53014-376-62 2.5 g unit dose foil packs, 144 per carton

Route:Spray

Side Effects: Stinging, burning, itching, tenderness, redness, dry, white flakes

Precautions: Pregnancy, nursing mothers, adolescence

Purpose: Relieves pain of lacerations

OB Purpose: Relieves pain of lacerations

Acetaminophen/codeine (Tylenol #3)

 

Definition

 Action: Opioid pain medication

 Dosage:300 mg

Route: Oral

Side Effects: Drowsiness, constipation, upset stomach, blurred vision, dry mouth

Precautions: Hypersensitivity, liver disease, opioid addiction

Purpose: Relieves moderate to severe pain

OB Purpose: Not to be used during pregnancy, can help the mother with pain after birth

Definition

Action: May inhibit prostaglandin synthesis, to produce anti-inflammatory, analgesic, and antipyretic effects

Dosage: Capsules: 200 mg Injection: 800 mg/8ml sing dose vials Oral drops: 40 mg/ml, 50 mg/1.25ml Oral suspension: 40 mg/ml, 100mg/5ml Tablets: 100, 200, 300, 400, 600, 800 mg

Chewable tablets: 50, 100mg

Route: Capsule, IV, oral drops, oral suspension, tablets, chewable

Side Effects: Dizziness, headanes, nervousness, edema, fluid retention, tinnitus, abdominal pain, bloating, constipation, decreased appetite, diarrhea, dyspepsia, epigastric distress, flatulence, heartburn, nausea, non-necrotizing enterocolitis, vomiting, acute renal failure, azptemia, cystitis, hematuria, agranulocytosis, aplastic anemia, leukopenia, neutropenia, pancytopenia, thrombocytopenia, anemia, prolonged bleeding time, hypokalemia, hypoglycemia, pruritis, rash

Precautions: Patients hypersensitive to drug and those with angioedema, syndrome of nasal polyps, or bronchospastic reaction to aspirin or other NSAIDs, treatment of perioperative pain after CABG surgery, NSAIDs can increase risk of heart attack in stroke patients with or without heart disease or risk factors for it

Purpose: To reduce symptoms of arthritis, treat mild to moderate pain, reduce fever

OB Purpose: Drug can cause fetal harm and is contraindicated in pregnant women starting at 30 weeks gestation. Before 30 weeks, use only if potential benefit justifies potential risk to fetus. Drug also appears in breast milk.

Definition

Action: May inhibit prostaglandin synthesis to produce anti-inflammatory, analgesic, and antipyretic effects

Dosage: Injection: 10 gm/ml in 1ml ampules, 15mg/ml in 1- and 2- vials, and 1ml

Tubex syringes: 30 mg/ml in 1- and 2-ml single-dose vials, 1- and 2-ml Tubex syringes, and 10ml multiple dose vials Nasal spray: 15.75 mg/spray Tablets: 10 mg

Route: PO, Injection, nasal

Side Effects: frequent : headache, nausea, cramps, dyspepsia

Precautions: Hypersensitivity to ketorolac, aspirin, or other NSAIDs. Intracranial bleeding, hemorrhagic diathesis, high risk of bleeding; concomitant use of aspirin; NSAIDs, labor and delivery, renal impairment, GI bleeding, etc.

Purpose: Analgesic, intraocular anti-inflammatory

OB Purpose: Avoid during 3rd trimester, precautioned for labor and delivery

Definition

Action: Opioid to relieve pain

 Dosage: 300 mg

 Route: Oral: Capsule, liquid, tablet

Side Effects: Nausea,vomiting, constipation, dizziness, mood changes

 Precautions: High risk for addiction

 Purpose:Relieve moderate to severe pain

 OB Purpose: None

Bisacodyl (Dulcolax Suppository)

 

Definition

Action: Stimulates intramural nerve plexi to have a direct effect on colonic smooth musculature

Dosage: 10 mg

Route: Rectal, suppository

Side Effects:Abdominal discomfort, nausea, cramps, faintness, burning of rectal mucosa, mild proctitis

Precautions: Hypersensitivity to bisacodyl, abdominal pain, appendicitis, intestinal obstruction, nausea, undiagnosed rectal bleeding, pregnancy, lactation

Purpose:Treat constipation, colonic evacuation before exams or procedures

OB Purpose: Pregnancy and lactation are contraindications

Anusol- HC Suppository

 

Definition

Action: Zinhibits accumulation of inflammatory cells at inflammation sites, phagocytosis, lysosomal enzyme release, synthesis, and/or release of mediators of inflammation. Reverses increased capillary permeability.

Dosage: 100mg/60 ml

Route: Rectal

Side Effects: Irritation, redness

Precautions: Hypersensitivity to hydrocortisone

Purpose: Treat hemorrhoids and itching in rectum and anus

OB Purpose: Can help mother with hemorrhoids after birth

Definition

Action: changing the surface tension of gas bubbles and causing collapse of foam bubbles, thus allowing easier passage of gas and preventing gas pockets in the gastrointestinal tract
