Present with dilatation of all 4 chambers of the heart, consequence of viral myocarditis (Coxsackie), Adriamycin toxicity (patient treated for soft tissue tumor), peripartum in women with gestation with multiple babies, carnitine deficiency, Chagas’ disease, Barth syndrome, Becker and Duchenne muscular dystrophy, cardiomegaly, very low systolic ejection fraction

Younger patient with syncope (fainting) or sudden death associated with significant exercise or exertion, genetic mutation of beta myosin heavy chain gene, asymmetric hypertrophy of interventricular septum, myofiber disarray in a storiform or pinworm pattern, cardiomegaly, normal systolic ejection fraction
