Definition
Immediately dry infant under warmer or skin-to-skin with mother, suction mouth first, then nose with bulb syringe, keep head slightly lower than body, assess airway status.

When does the nurse obtain Apgar Scores?
Definition

 

At 1 and 5 minutes after delivery. A hands-on examination. Scores given of 0-2. Do NOT wait until 1 minute Apgar is assigned to begin resuscitation of compromised neonate.

 

7-10=good

4-6=needs moderate resuscitative efforts

0-3= severe need for resuscitation

What does Apgar Assessment measure?
Definition

HR: Absent=0, Under 100=1, 100 or >=2

 

Respiratory effort: No cry=0, Weak cry=1, Vigorous cry=2

 

Muscle tone: Flaccid=0, Some flexion=1, Total flexion=2

 

Reflex irritability= No response to tap=0, Slight response=1, quick foot removal=2

 

Color= Dusky/cyanotic=0, Acrocyanotic=1, Totally pink=2

Definition

Indications/Prevent ophthalmia neonatorum/chlamydia

Adverse reactions/ Most commonly used agent, puffy eyes from manipulation

Nursing implications/Thin line along entire lower lid in conjunctival sac, one tube per baby and discard, manipulate eye to ensure complete coverage, may wipe excess after one minute

Definition

Indications- Prevention of ophthalmia neonatorum from gonorrhea expoure through birth canal in vag delivery

 

Adverse reactions- Red,puffy eyes, staining of skin if contact occurs

 

Nursing implications- May not kill other organisms such as Chlamydia, 2gtts in lower congunctival sac making sure drops spread over entire eye, DO NOT irrigate eyes following instillation

How to give IV meds during labor,
Definition
Push IV bolus in line slowly, at beginning of contraction, give med during contraction, when uterine blood vessels are contricted, so less reaches to fetus.
