Term
Hyperkeratosis
Definition
thickened keratin layer of the surface epithelium
Term
hyperparakeratosis
Definition
there is no granular cell layer and the epithelial nuclei are retained in the keratin layer.
Term
spongiosis
Definition
intercellular edema of the spinous layer
Term
acanthosis
Definition
thickened spinous layer
Term
epithelial dysplasia
Definition
alteration in size, shape, and organization of adult cells. Usually begins in the basilar portions of the epithelium.
Term
verrucous hyperplasia
Definition
a type of papillary, exophytic proliferation of squamous epithelium. Usually seen in smokeless tobacco
Term
leukoedema
clinical features
Definition
*white bluish tinge of the buccal mucosa
*painless and persistent
*more common in blacks and dark skinned caucasians
*not a neoplasm but a normal variation
**white appearance disappears when tissue is stretched (differentiates from leukoplakia)
Term
linea alba
features
Definition
*white line along buccal mucosa in region of occlusal plane
*5-13% of all patients
*due to biting cheeks
*differentiate from moricatio buccarum
Term
squamous papilloma
clinical feat
Definition
*COMMON oral lesion
*exophytic, "cauliflower" like soft lesion
*1/3 found of soft palate or uvula.
Term
Squamous papilloma
Histology
Definition
long thin finger like projection of squamous epithelium containing a thin connective tissue core
*may show hyperkeratosis
Term
tx of Squamous Papilloma
Definition
excisional biopsy (including base of lesion), vaccine targeted for HPV tyes 6, 11, 16, and 18
Term
verrucous vulgaris
clinical features
Definition
common painless persistent
wart-like with sessile base
FINGER LIKE PROJECTION (which always comes down to a squamos core)
white or normal color
can be clinically identical to papilloma
Term
verrucous vulgaris
histology
Definition
pointed projections from the surface wpithelium
thickened keratin
viral inclusions may be seen in upper spinous layer
inflammation in connective tissue
Term
dysplastic oral warts
Definition
ranges from dysplasia to carcinoma in situ
HPV subtypes 16, 18
Term
condyloma acuminatum
vs
squamous papilloma
Definition
condyloma acuminatum grows faster and gets larger- much faster proliferation
CM is mainly due to HPV 6, 11
incubation period for CM is 1-3 months
Term
condyloma acuminatum
clinical features
Definition
sexually transmissible and autoinoculable viral disease (STD)
may occur 1-3 months after contact
soft papillary nodules that proliferate rapidly in size
most common in anogenital skin
if in mouth:
*multiple papillary masses involving tongue, buccal mucosa, palate, gingiva
*white or pink
*up to 2cm in diameter
**in anogenital area, HPV 16 &18 are associated with increse in squamous carcinoma
Term
Focal Hyperplasia (Heck's Disease)
Clinical Features
Definition
painless and persistent
ist observed in american indians
flattened multiple nodular lesions usually on lower lip (also on commissures, buccal mucosa, upper lip and tongue)
lesions usually 1-5mm diameter
same color as adjacent mucosa
may undergo spontaneous regression after 4-6mo or years
Term
Focal Hyperplasia
Histology
Definition
ancanthosis
mitosoid cells are simetimes seen in squamous epithelium
Term
verruciform xanthoma
clinical features
Definition
lipid laden histocytes beneath epithelium
uncommon, painless, persistent
usually solitary lesion up to 1.5cm diameter
sessile or pedunculated base
ROUGH or PEBBLY surface
Term
Verruciform xanthoma
HISTOLOGY
Definition
verrucous hyperparakeratotic surface with elongated rete ridges
LARGE SWOLLEN FOAM CELLS (HISTOCYTES AND XANTHOMA CELLS) in connective tissue papillae between peithelial pegs
Term
keratoacanthoma (self-healing carcinoma)
clinical features
Definition
*self healing benign lesion that clinically resembles squamous cell carcinoma
usually on sun exposed skin
may be: solitary or multiple, painful
3 growth phases: growth, static, involution
Term
keratoacanthoma
histology
Definition
abnormal keratin production
Term
keratoacanthoma
histology
Definition
abnormal keratin production
Term
white spongus nevus
clinical features
Definition
autosomal dominant transmitted condition.
due to defect of normal keratinization
mucosa appears thickened and corrugated with sponge texture
usually involves buccal mucosa (also palate, gingiva, floor of mouth, and tongue)
childhood onset
often mistaken for leukoplakia
Term
white sponge nevus
histology
Definition
acanthosis and hyperparakeratosis
intracellular edema on surface
Term
leukoplakia
Definition
white patch on the mucosa that will not wipe off and is no other specific disease
ONLY A CLINICAL TERM
Term
frictional (focal) keratosis
Definition
non cancerous
similar to callus on skin
Term
leukoplakia
clinical features
Definition
premalignant lesion
older population
varies: small localized area to large diffuse lesion (multifocal)
vary from non palpable faintly white to leathery areas
most common sites are BUCCAL MUCOSA AND MANDIBULAR ALVEOLAR MUCOSA
80% are smokers
Term
clinical types of leukoplakia
Definition
mild or thin
homogenous or thick
granular or nodular-bad
verrucous or verruciform-bad
Term
histology of leukoplakia
Definition
acanthosis, hyperkeratosis and hyperparakeratosis
possibly some mild inflammation
***may become dysplastic, invasive without any change in clinical appearance.
