HIV+ patient with problems hearing for the past month:
Definition
- Serous otitis media, the most common middle ear pathology for pts c AIDS.
- Due to auditory tube dysfunction arising from HIV lymphadenopathy or obstructing lymphomas.
- Characterized by middle ear effusion without evidence fo an acute infection. Exam: dull TM that is hypomobile on pneumatic otoscopy.
Term
12 year old with left ear discharge and recent unsucessful treatment for otitis media, with hearing loss. On exam, TM is intact, but ther eis peripheral granulation and some skin debris. Dx?
Definition
- Cholesteatoma
- These can be either congenial or aquired, with congenital lesions found around the age of 5. Aquired usually occur 2/2 chronic middle ear disease.
- Chronic middle ear disease leads to the formation of a retraction pocket in the TM, which can fill with granulation tissue and skin debris.
- Complications include hearing loss, CN palsies, vertigo, and serious infections such as brain abscesses or meningitis.

Definition
accumulation of fluid in the inner ear that leads to hearing loss, vertigo, and tinnitus.

The most dangerous space for a pharyngeal abscess:
Definition
The retropharyngeal space. The most feared complication of this is spread to the mediastinum, which can lead to acute necrotizing mediastinitis.
