'Who' refers to a person
'That' refers to an object or for a restrictive clause that would change the meaning of the sentence without it. (no comma with 'that')
'Which' is used with a nonrestrictive clause that doesn't change the meaning of the sentence. (use comma's with 'which')

And, but, or, so, nor, yet

The comma falls before the conjuction, when the conjunction is in the middle of the sentence. When it's in the beginning of the sentence (transitional word), the comma falls after the conjunction.

In the case of coordinating conjuctions, when should a comma be used?
Definition
When there are 3 or more segments AND a coordinating conjunction are used. *If there is already a comma in the sentence, use a semicolon.
