Term
if the beat originates in the Atria or AV node then the QRS is usually:
Definition
narrow, if it originates in the ventricals then it is wide and bizarre
Term
4 general groups of arhythmias
Definition
1. irregular rhythms
2. escape/premature beats
3. rapid ectopic rhythms
4. atrioventricular heart blocks
Term
p waves and p-r intervals are identical but rate varies:
Definition
sinus arrhythmia
Term
p waves of varying shape and different p-r intervals
Definition
wandering atrial pacemaker OR if rate >100 then its multifocal atrial tachycardia
Term
no p waves; irregular/wavy baseline and QRS is irregularly spaced:
Definition
atrial fibrillation
Term
usual pacemaker fails so a slower pacemaker fires at its inherent rate:
Definition
escape beat: could be atrial, junctional or ventricular
Term
escape beat: different appearing and late p waves
Definition
atrial escape beat
Term
escape beat: no p waves normal narrow QRS
Definition
junctional escape beat
Term
escape beat: no p wave and wide bizarre QRS
Definition
ventricular escape beat
Term
ectopic pacemaker fires early before the next scheduled beat:
Definition
premature beat
Term
pwave may run into previous T wave; caused by adrenergics
Definition
premature atrial contraction
Term
early beat with no p wave and normal narrow QRS:
Definition
premature junctional beat
Term
early beath with no p wave and wide QRS; caused by (3):
Definition
premature ventricular contraction
caused by: hypoxia, hypokalemia, mitral valve prolapse
Term
accelerated rate (150) and narrow QRS:
Definition
paroxysmal supraventricular tachycardia
Term
accelerated rate and wide QRS:
Definition
ventricular tachy
Term
large eratic and wavy baseline:
Definition
ventricular fibrillation
Term
PR interval >0.2:
Definition
1st degree AV heart block
Term
PR interval gets progressively longer until a QRS is dropped:
Definition
2nd degree AV heart block = Wenkebach
Term
PR intervals are constant and QRS is dropped intermittently:
Definition
Mobitz; 2nd degree AV heart block
Term
PR interval is constantly changing and QRS is wide:
Definition
3rd degree AV block (most serious)
Term
QRS interval should be less than:
Definition
<.12
Term
QRS >.12 =
Definition
bundle branch block
Term
QRS >.12 and RR in V1-V2 =
Definition
Right BBB
Term
QRS >.12 and RR in V5-V6 =
Definition
Left BBB
Term
VERY IMPORTANT: The EKG is often unreliable in detecting ________ during LBBB.
Definition
infarct
Term
QT should be less than :
Definition
1/2 R to R interval
Term

Definition
Junctional Rhythm: no p waves, relatively narrow QRS
Term
[image]
Definition
First Degree Block
PR interval is greater than .2 seconds (one large box). Note that there is a P wave before each QRS and a QRS after each P wave
Term
[image]
Definition

paroxysmal supraventricular tachycardia

Regular narrow QRS. Often it's hard to determine whether it's atrial or junctional as it is going too fast to see P waves. Clinically it doesn't matter since they are treated the same

Term

Definition
Normal Sinus Rhythm
Term

Definition

LBBB

Left Bundle Branch Block
The wide QRS in V5 and V6; the RR' is present but not as distinct as in right bundle branch block. With left bundle branch block the signs of infarction are unreliable

Term

Definition
junctional rhythm
Term

Definition
Atrial fibrillation
Term
[image]
Definition
Third Degree Block
The atrial rate is independent of the ventricular rate so the P waves are independent of the QRS's. The P waves are regular and can be marched out with calipers. The same can be done with the QRS complexes
Term
[image]
Definition

Second Degree Block Type 2
PR intervals are the same, but every other beat is dropped

Term
[image]
Definition
Ventricular tachycardia
Term
[image]
Definition
Second Degree Block Type 1 (Wenckebach)
The PR gets progressively longer until a beat is dropped
Term
[image]
Definition
Wandering Atrial Pacemaker
(It's called Multi focal Atrial Tachycardia if the rate is >100). Strict definition is 3 different P wave morphologies and 3 different PR intervals
Term
[image]
Definition

PAC

Premature Atrial Contraction

The third beat is premature. It has a P wave of different morphology than the sinus beats. This defines it as a premature atrial contraction

Term

Definition
RBBB
Term
[image]
Definition

sinus arrhythmia

Irregular, but note P wave morphology and PR intervals are identical, this identiies it as a sinus rhythm.

Term
atrial rate >100, assoc w COPD and sometimes Digoxin toxicity
Definition
multifocal atrial tachycardia
Term
what are the rates assoc with ea rhythm? paroxysmal tachy, flutter, fibrilation
Definition
paroxysmal tachy:150-250
flutter:250-350
fibrilation:350-450
Term
o Very rapid ventricular rhythm caused by hypokalemia or congenital d/o
o 250-350 bpm
o Looks like twisted ribbon, get progressively larger than smaller
o Usually brief self-terminating bursts, but if unresolved can lead to deadly arrhythmia
Definition
- Torsades de Pointes
Term
o AKA Barlow’s syndrome, benign condition, more common in women
Definition
Mitral Valve Prolapse
Term
PAC w widened QRS:
Definition
PAC w aberrant ventricular conduction
Term
atrial bigeminy:
Definition
irritable atrial focus repeatedly couples a PAC to the end of each normal cycle
Term
atrial trigeminy:
Definition
2 normal cycles followed by a PAC
Term
bundle of kent:
Definition
an extra or accessory conduction pathway between the atria and ventricles in the heart - assoc w Wolfe-Parkinson-White syndrome
Term
Wolfe-Parkinson-White Syndrome:
Definition
premature ventricular contraction caused by short circuiting from the bundle of Kent
Term
conduction tract in left atrium. Originates in the SA node and distributes depolarization.
