Definition

Implantation of the fertilized egg outside of the uterus.

 

Approx 1% of pregnancies due to fertility treatments, STDs and PID and use of IUDs.

 

Presents with bleeding and unilateral pain.

 

Dx with palpation of an adnexal mass or US

 

Tx is surgery pr methotrexate

usually secondaryto uterine or cervical abnormalities, trauma, systemic disease or infection

 

D&E, prostaglandins, oxytocin used for mgmt of spontaneous abortion

 

Risk of uterine perforation with D&E is greater in 2nd trimester

Definition

Painless dilation and effacement often in the 2nd trimester

 

If previable (pess than 24 weeks) expectant mgmt or termination are options

 

If viable: strict bedrest, and betamethasone to decrease risk of prematurity.

 

Tocolysis and cerclage are also available.
