ASA (aspirin)
Definition
  • Antiplatelets
  • COX1 and COX2 inhibitor
  • reduce platelet aggregation
  • T1/2= 15 min to 12 hrs
  • onset 5-30 min, duration 3-6 hrs
  • analgetic and antiinflammatory
  • reduces the risk of recurrent TIA, CVA, MI
  • ADR: GI erosion, decreased renal blood flow, bleeding, bronchoconstriction (5-10%), Reye's syndrome in peds
  • metabolized in the liver, excreted in urine

  • Antiplatelets
  • ADP receptor inhibitor
  • prodrug, active form in the liver
  • may be inhibited by omeprazole
  • stomach distress
  • thrombocytopenic purpura (TTP) within the first 2 weeks of tx
  • aplastic anemia, bleeding
  • 75 mg daily, starting dose 300 mg x1

Definition
  • Antiplatelets
  • PDE (phosphodiesterase) inhibitors - increase cAMP
  • metabolized in liver, excreted in bile
  • t1/2=10 hrs
  • used in combination with warfarin for thromboembolism prophylaxis
  • ADP: GI disturbance syncope, hypotension, rash
  • 75-100 mg QID or 150-400mg/d with ASA

  • PDE inhibitors
  • used only for peripheral arterial disease, intermittent claudication
  • vasodilation, increase HDL and decrease triglycerides
  • 12 for full effects to develop
  • reverse 48 hrs after withdrawal
  • metabolized in liver
  • inhibited by omeprazole
  • Contraindicated - HF, active bleeding, hemostatic disorders

  • block thrombin-induced platelet activation
  • long t1/2= few days - thus the final effect is irreversible
  • approved May 2014 - expensive
  • USED pts h/o MI and PAD
  • NOT RECOMMENDED pts h/o stroke, TIA, intracranial hemorrhage
