Activated Charcoal
Definition

Class:Absorbent

Action:absorbs toxic substances from GI tract

Indications:oral posionings and med overdoses

Contraindications:oral admin to comatose patients,  ingestion of corrosives, caustics, petroleum distillates

Side fx:aspiration can induce fatal form of pnuemonitis, constipation, black stools, diarrhea, vomiting

Route/dose:Adult:1 to 2g/kg PO or NG tube. Pedi: Same as adult

Supplied:25g (powder)/125ml bottle or 50g(powder)/250ml bottle

Special:must be stored in closed container, pregnancy safety

Adenosine

(Adenocard)

Definition

Class:Antidysrhythmic

action:Slows conduction through AV node, slows heart rate by acting directly on the sinus pacemaker cells by slowing impulse formation

Indications:conversion of PSVT to sinus rhythm. Not effecive in converting Afib or V-tach.

Contraindications:22nd or 3rd degree AV block, sick sinus syndrome, bronchoconstrictive or bronchospastic lung disease. drug induced tachycardia.

Side fx:generally short duration and mild;headache, dizziness, dyspnea, chest pain, hypotension, bronchospasm, dysrhythmias, cardiac arrest, diaphoresis

Route/Dose: Adult:6mg rapid IV bolus over 1-3 seconds, followed by 20 ml flush and elevate extremity. No response after 1-2 minutes, 2nd dose or 12 mg rapid IV bolus over 1-3 seconds. Pedi: 0.1mg/kg rapid IV bolus, 5-10ml flush. second dose 0.2mg/kg, then flush

How supplied:3mg/ml in 2ml and 5ml flip top vials

Special considerations: Pregnancy safety, may cause Bronchoconstriction in asthma patients, evaluate elderly for dehydration

ALBUTEROL
Definition

class:bronchodialtor, sympathomimetic

action:selective beta 2 agonist, stimulates adrenergic receptors of sympathomimetic nervous system

indications:bronchospasms in patients with COPD

contraindications:known prior hypersensitivity reactions to albuterol, tachycardia and dysrhythmias

side fx:headache,fatigue, light headedness, aggressive behavior, pulmonary edema, chest pain

route/dose:Adult:2.5mg, dilute in .5ml of solution for inhaltion with 2.5ml NS, metered dose inhaler. Pedi 1.25 mg/dose hand held nebulizer

how supplied:metered dose inhaler

special considerations:may precipitate angina pectoris and dysrhythmias

Definition

Class:antidysrhythmic

action:blocks sodium channels and myocardial potassium channels

indications:v-fib, pulseless v-tach, unstable v-tach

contraindications:known hypersensitivity to amiodarone or iodine, cardiogenic shock, sinus bradycardia, severe sinus node dysfunction

side fx:dizziness, fatigue, malaise, tremor, ataxia, respiratory distress, CHF, bradycardia, hypotension

route/dose:Adult:300mg IV/IO push, following dose 150mg, pedi:5mg/kg IV/IO Bolus

How supplied:50mg,ml vials and syringes

special considerations:Hypotension, may worsen dysrhymias

Definition

Class:antidote

action:converts hemoglobin to methemoglobin

indications:cyanide poisoning
Contraindications:none in emergency setting

Side fx:headache, dizziness, weakness, ICP, shortess of breath, tachycardia, syncope

route/dose:Adult:1-2 ampules, inhaled for 30 seconds, pedi:1 ampule

How supplied:0.3ml ampules

Special considerations:flammable, hypotensive effects

Class:platelet inhibitor, anti-inflammatory agent

action:prevents clotting

Indications:new onset of chest pain, MI

Contraindications:hypersensitivity, asthma

side fx:bronchospasms, anaphylaxis, nausea, vomiting

route/dose:Adult:160 mg-325mg tablet, Pedi:not recommended

how supplied:81mg, 160mg and 325mg tablets

special considerations:not recommended in pedi patients

Definition

Class:beta blocker, antidystrhythmic

action:decreases heart rate, myocardial contractility and cardiac output

indications:to reduce myocardial ischemia and damage in MI patients

Contraindications:heart failure, cardiogenic shock, bradycardia

side fx:dizziness, bronchospasmss, bradycadia, hypotension

route/dose:Adult:5mg SIVP, not recommended in Pedi

how supplied:5mg in 10ml ampules

special considerations:Slowly over 5 minutes, caution in pts with liver or renal dysfunction

Atropine Sulfate
Definition

Class:anticholinergic agent

Action:inhibits action of acetylcholine at parasympathetic neueroeffector sites. increases heart rate in life threatening bradycardia

indications:hemodynamically unstable bradycardia, organophospate poisoning, beta blocker or calcium channel blocker overdose

contraindications:Tachycardia, hypersensitvity, unstable cardiovascular status in acute hemorrhate with myocadial ischemia

side fx:drowsiness, confusion, headache, tachycardia, palpitations, nausea, vomitting

route/dose:Adult 0.5mg IV/IO, Pedi: 0.02mg/kg

how supplied:pre-filled syringes

special considerations:pupillary dilation, parodical bradycardia may occur
