Definition
  • synthetic catecholamine, primarily stimulates beta 1 adrenergic receptors
  • positive inotropic, minimal chronotropic activity or systemic vascualr resistance
  • useful in management of CHF when increased heart rate is not desired
Term

Dobutamine - onset and duration

Definition

onset: 1-2 minm peak after 10 min

duration: 10-15 min

Term

Definition
inotropic support for pt's with left ventricular dysfunction

Definition
  • tachydysrhythmias (a-fib, a-flutter)
  • severe hypotension with signs of shock
  • idiopathic hypertrophic subartic stenosis
  • suspected . known drug-induced shock
Term
Dobutamine - adverse reactions
Definition
  • headache
  • hypotension
  • dose related tachydysrhythmias
  • ventricular ectopy
Term
Dobutamine - drug interactions
Definition
  • beta adrenergic antagonists - may blunt inotropic response
  • sympathomimetrics and phosphodiesterase inhibitors - may exacerbate dysrhythmia response
  • incompatible with sodium bicarb and furosemide - forms ppt - use different line
Term
Dobutamine - supply
Definition
12.5 mg/ml injectable
Term

Dobutamine - dosage and administration

Adult

Definition
2-20mcg/kg/min IV, based on inotropic effects
Term

Dobutamine - dosage and administration

pediatric

Definition
2-20mcg/kg/min IV/IO, titrate to effect
Term
Dobutamine - special considerations
Definition
  • preg. cat. : C
  • admin via infusion pump
  • may be admin through y site concurrent with dopamine, lidocaine, nitro pusside, KCl
  • monitor BP closely
  • increased heart rate more than 10% may induce/exacerbate MI
  • have lido readily available
  • correct hypovolemia before admin in hypotensive pt's
