  • oldest most widely used TCA
  • 1st gen. antidepressant
  • highest risk of suicide because; NO antidote for OD or poisoning
  • SE:  dysrythmias, seizure, headache
  • caution: w/ hepatic, renal & cardiac dysfunction
  • no concurrent use w/ MAOI, St. Johns wort due to hypertensive crisis and possible death
  • Monitor: BP, and cardio
  • up to 4wks for results. give @ night due to sedative effect
  • **DON'T stop abruptly**

  • SE:  dizziness, tremors, nervousness, insomnia, N/D, constipation, dry mouth, sexual dysfunction, weight loss/gain,
  • HIGH RISK suicide under 18yrs old**

**Precaution if seizure or head injury as it lowers threshold

  • alters cation transport in nerve & muscle
  • treat manic episodes
  • Therapeutic levels:
  • Manic phase: 1-1.5 mEq/L
  • maintenance: 0.6-1.2 mEq/L

Toxicity level >1.5mEq/L

Must have adequate Na+ in diet to avoid lithium toxicity

2,500-3,000mL fluid daily

  • ↑GABA in the brain
  • reduce manic episodes, adjunct to schizophrenia
  • SE:  sedation, drowsiness, N/V/D/constipation, dizziness, hepatic failure
  • must wean off slowly to prevent seizures; can cause physical dependency
  • **therapeutic range is 50-100**
