Term
What are the 7 major goals of PT rehab of burns?
Definition
Promote wound closure, prevent infection, reduce edema, manage pain, prevent loss of ROM, promote functional independence, and educate the pt.
Term
What must the PT be careful of with burn pts and hydrotherapy?
Definition
That they are very gentle, the agitators are not turned on, that the water temp is at body temp.
Term
What are the benefits of hydrotherapy with a burn pt?
Definition
It can aid in dressing removal, they can do exercises while in the tank more effectively because the bandages are removed, and they are less painful in the tank.
Term
What is important to remember when applying topical agents to a burn wound?
Definition
To keep contact minimal!
Term
How is massage done in the burn pt when trying to decr. edema? (the technique)
Definition
Massage in proximal to distal segments, but the direction of the massage segments is distal to proximal. ie: massage from upper arm to shoulder in a proximal diretion. Then massage from the elbow to the upper arm and shoulder. then from mid forearm to upper arm, etc...
Term
How useful is TENS in decr. pain in the burn pt?
Definition
It isn't. Their pain is too high for this to have an effect.
Term
How should ROM exercises be performed with the acute burn pt?
Definition
It should be AROM only. No stretching!
Term
What is the collegen:elastin ratio of scar tissue? What is the impact of this in relation to normal skin?
Definition
it is high. (so there's more collagen than elastin) this makes the scar tissue more stiff, and less elastic than normal skin tissue.
Term
How do you facilitation proper alignment of collagen fibers in the proliferative phase of burn wound healing?
Definition
Gently stretch surrounding skin so the direction of force is in the direction of optimal alignment.
Term
How would you splint a burn pt w/ an axilla burn during the proliferative phase of wound healing?
Definition
Spint in abd to prevent add contracture.
Term
What happens if during the maturation phase of burn healing, the synthesis of tissue is faster than the lysis of tissue?
Definition
The pt will develop a keloid or hypertrophic scar.
Term
What is scar maturation dependent on? (8)
Definition
Race, genetic predisposition, burn depth, type of graft, burn size, age and location.
Term
How can you prevent web space contractures in the pt w/ a hand burn?
Definition
Wrap fingers individually and use foam wedges.
Term
How can the pigmentation of the skin affect scar formation?
Definition
Very fair and very dark skinned are at higher risk for keloid scar formation.
Term
What are your 6 major goals for rehab of the burn pt?
Definition
Gain ROM, Decr edema, Incr strength, Decr risk of undesirable functional and cosmetic scar hypertrophies, Maintain functional independence, Independence donning and offing of all devices.
Term
What are some important points of pt education to protect their skin? (3)
Definition
White, scent-free lotion should be used on the skin, they must test bathwater and shower temp until 1.5yrs, they should avoid exposure to cold and heat.
