Increases the amount of blood circulating to vital organs via peripheral vasoconstriction, increases the chance of the heart returning to a normal rhythm, strengthens cardiac contractions, increases HR and BP.

Indications: Cardiac Arrest: VF, pulseless VT, asystole, PEA

IV/IO Dose: 1 mg every 3-5 minutes of 1:10,000 solution (0.1 mg/mL [1 mg/10 mL])

Acts as an activator in the transmission of nerve impulses and contraction of cardiac, skeletal, and smooth muscle.

To stabilize the contraction of cardiac tissue after metabolic changes have caused instability and arrhythmias

1 g of 10% Calcium Chloride (100 mg/mL [give 10 mL])

or

2-4 mg/kg, may repeat in 10 min if needed

Definition
Given during cardiac arrest to treat ventricular arrhythmias, mainly VF (ventricular fibrillation) and VT (ventricular tachycardia) after defibrillation has failed. The overall effect is to slow the heart (sinus rate).

IV/IO 1st dose 300 mg, may repeat once after 3-5 min with 150 mg

lidocaine HCl
Definition
Alternative to amiodarone as an antiarrhythmic in cardiac arrest from VF/VT.
(Suppresses automaticity and spontaneous depolarization of the ventricles during diastole by altering the flux of sodium ions across cell membranes with little or no effect on heart rate.)

Initial dose: 1-1.5 mg/kg IV/IO; repeat in 5-10 minutes; max 3 doses or total of 3 mg/kg

1 amp = 100 mg in 5 mL (2%)

or

1 - 1.5 mg/kg bolus, may repeat doses of 0.5 - 0.75 mg/kg q 5-10 min up to a total dose of 3 mg/kg, may then start continuous infusion of 1-4 mg/min

Increases HR. Anticholinergic. Antimuscarinic. Antiarrhythmic.

IV: 0.5 - 1 mg q 3-5 min, not to exceed a total of 2 mg or .04 mg/kg

Adjunct to standard measures to improve/increase BP & CO

IV:
Beta-adrenergic (cardiac stimulation): 5-15 mcg/kg/min
Alpha-adrenergic (increased peripheral vascular resistance): >15 mcg/kg/min

Titrate infusion to patient response / increase as needed.
