        | Adult: VF/Pulseless VT: 300 mg IV/IO push. Initial dose can be followed once in 3-5 minutes at 150 mg IV/IO over 10 minutes; may repeat every 10 minutes if need. After conversion, follow with 1 mg/minute infusion for 6 hours, then a 0.5 mg/minute maintenance infusion for 18 hours. Max cumulative dose: 2.2 g IV/IO per 24 hours   Pediatric: Refactory VF/Pulseless VT: 5 mg/kg IV/IO bolus. Can repeat the 5 mg/kg IV/IO bolus up to total dose of 15 mg/kg per 24 hrs. (2.2 g in adolescents). Max single dose: 300 mg Perfusing ventricular or atrial dysrhythmia loading dose 15 mg/kg per day, (2.2 g in adolescents) |  |
