STATIC against G+/G-

Undergo enterohaptic recycling - persistant Rx level

Mechanism:  inhibit binding of aminoacyl tRNA to acceptor site

Short-acting (HL = 6-8 h)

Alt oral/topical for acne

Intermediate Acting (HL = 12 h)

Use: syndrome of inappropriate ADH (vasopressin) (SIADH)

Mechanism of SIADH:  demeclocyline inhibits protein kinase in ducts, prevent ADH --> promotes water secretion & plasma Na level to normal

Long Acting (HL = 16-18 h)

Oral & IV

Use: acne

Use:  acne assoc w/ pulmonary complications (pulm. lupus, hypersensitivity pneyumonitis, pleural effusions)

Positive traits:

Immunomodulatory & anti-inflamm: blocks synthesis of NO, microglia activation --> DEC neuronal damage (improved stroke outcome)

Neuroprotective in Parksinson, Huntinton's, ALS, etc.

Side Effects:  SLE-like syndrome, rare chronic use: nail, skin & sclera pigmentation (black thyroid, blue-black gums)

• Slowed growth
• Brown teeth (permanent)
• Photosensitivity
• FANCONI-LIKE syndrome (fr expired Rx) fr nephrotoxic metabolic epianhydrotetracycline --> nausea, vomiting, polyuria, polydispia, glyucosuria

Rx Interaction:  Absorption DEC w/dairy products and antacids (mechanism: chelation - binding/inactivating)

Similar side effects, mechanism as tetracyclines

Against more tetracycline-sensitive G+/G- and MRSA

Use: complicated skin, intraabd. infections, CABP

Side Effects: N&V

FDA - INC death, esp hospital acq. ventilator assisted pneumonia
