  • Cholinesterase Inhibitor (Cholinergic = M and NM receptors) - Blocks the action of the enzyme that breaks down ACh... allowing ACh to have it's affects longer
  • Reversibly inhibits AChE, with tx affecting mAChR's and nAChR's only
  • Tx: Myesthenia Gravis, reverses effects of Tubocurarine
  • SE's: Excessing mAChR activation, ACh remains in NMJ for too long = respiratory paralysis

Tubocurarine
Definition
  • Competative Neuromuscular Blocker (non-depolarizing)
  • Competeatively inhibits ACh from binding to Nm receptors, NO CNS effects due to + charge, no access across BBB(thus pain/concious remains)
  • Tx: Causes flaccid paralysis, used for mechanical ventilation, endotracheal intubation, surgery
  • SE's: Resp. depression, dec. BP, dec. HR - all reversed by AChE inhibitor

  • Depolarizing neuromuscular blocker
  • Short acting (compared to tubocurarine), binds to Nm, initially activating receptor-causing contraction, does not readily release from Nm, blocking ACh from binding
  • Tx: Muscle relaxation (muscle paralysis), used for short procedures (endotracheal intubation, endoscopy
  • SE's: malignant hyperthermia, post-op muscle pain, does not cross BBB due to + charge.

  • a1 vasoconstriction: dec absorption, bleeding, decongestion; inc BP; b1 restores cardiac funtion; b2 inc bronchodilation
  • HTN, angina pectoris, dysrhythmias, hyperglycemia in diabetics

  • SE's: Dec HR, bronchoconstriction, glycogenolysis, CAN cross BBB, so CNS is affected
