the Paramesonephric ducts failed to fuse. This is a bicorunate uterus (partially septate uteus w/ single cervix/vagina)   Normally they fuse to form uterine tubes, uterus cervix, and 1/3 vagina. Failure along there can cause at worst two separate uterine horns, cervixes, and two separate vaginas.
