Cards

Term
Abdominal Paracentesis
Definition
1. puncture of the peritoneum cavity with removal of fluid
2. pt. needs to empty bladder before procedure. The amount of fluid and charactertistics are documented.
Term
Angiogram / Arteriogram
Definition
1. Radiographic record of the size, shape, and location of the heart and blood vessels after introduction of a contrast media. A catheter is inserted into a peripheral vessel and guided to the affected area.
Term
Arthroscopy
Definition
1. Direct joint visualization by means of an arthroscope usually to repair, remove, or replace tissue.
2. Baseline data of pt. needed for this procedure. (ROM, girth measurements)
3. Postoperative - monitor VS, IV and oral fluids, assess neurovascular status, assess drainge devices, teaching for postoperative care (ice, swelling, joint soreness, weight bearing exercises, crutches, pain)
Term
Barium Enema / Lower GI Series
Definition
1. Introduction of a barium sulfate solution into the rectum as a contrast agent to facilitate x-ray and fluoroscopic examintaion of the colon (used to screen pts. for colon cancer or to identify lesions in diverticulitis)
2. Careful preparation of the bowel with laxatives and enemas is critical to eliminate retained feces
Term
Bone Marrow Biopsy
Definition
1. Removal of a small amout of tissue from the central core of a bone. Used to diagnose blood disorders, cancer, and infectious diseases (also can collect bone marrow for a bone marrow transport)
2. Advise pt. that some discomfort will be felt during procedure. (popping or crunching sound may be heard) Pt. must remain still during the procedure. Pt. needs to assume correct position, breath deeply, and practive relaxation techniques. Throughout procedure pt. is assessed for pallor and diaphoresis. Pain control measures are taken after the procedure.
3. Pt. needs educated on signs of infection. Pt. needs to avoid drugs containing aspirin because it can potentiate bleeding.
Term
Bronchoscopy
Definition
1. Examination of the bronchi through a bronchoscope.
Term
Cardiac Catheterization
Definition
1. Percutaneous intravascular insertion of a catheter into the cardiac ventricles, coronary arteries, or great vessels for disgnosis, assessment, inerventions, or evaluation of the effects of pathology on the heart and great vessels.
2. Assess VS for baseline and presence and intensity of peripheral pulses. Apply cardiac leads to pt. Be careful of pt. allergy to shellfish or iodine becasue of dye. Groin is cleansed and hair removed. Nitroglycerin may be given to aid in visualization. Pt. may feel warm, nauseated, or light-headed after the dye is inserted. Pt. wil have to lie on back for several hours afer the procedure and should repot chest pain during or after procedure.
3. Monoitor VS every 15 minutes for 1-2 hours post-op. The BP should not be checked in any limb used for catheter insertion. Dressing is checked often for bleeding and pt. needs to report any tightness.Pt. should avoid flexion of limp used for catheterization. The HOB is elevated no more than 30 degrees. Fluids given to flush out of dye so urinary frequency is common in pts. Pt. assessed for a variety of complications. Pt. needs to be driven home and a responsible adult should stay with them.
(You might want to read more on Taber's Dictionary - this was very lengthy so I just did the highlights.)
Term
Cholangiography
Definition
1. Radiography of the bile ducts, a procedure replaced by ultrasonography.
Term
Cholecystogram
Definition
1. A radiograph of the gallbladder. This procedure is being replaced by ultrasonography.
Term
Colonoscopy
Definition
1. Visualization of the lower GI tract. Most often refers to insertion of a flexible endoscope through the anus to inspect the entire colon and terminal ileum.
Term
CT Scan (Computerized Tomography)
Definition
1. Also abbreviated as CAT. (Computerized Axial Tomography)
Term
Cystography
Definition
1. Radiograph of any cyst into which a contrast medium has been instilled. (commonly used in the bladder)
Term
Cystoscopy
Definition
1. Examination of the bladder with a cystoscope.
Term
Doppler Studies
Definition
1. Uses ultrasound waves which are produced by something that is in motion. The use of Doppler effect permits measuring the velocity of that which is being studied (ex. blood flow in a vessel)
Term
Echocardiogram
Definition
1. The graphic record produced by echocardiography.
(echocardiography - a noninvasive disgnostic method that uses ultrasound to visualize cardiac structures. The heart's valves, walls, and chambers can all be evaluated, and intracardiac masses or clots can often be seen)
Term
EEG (electroencephalogram)
Definition
1. A tracing of an electroencephalograph
(electroencephalography - Amplification, recording, and analysis of the electrical activity of the brain. Electrodes are placed on the scalp in various locations and the difference between the two electrical sites is recorded) - commonly used in epilepsy and convulsant disorders
Term
EKG / ECG (electrocardiogram)
Definition
1. A record of the electrical activity of the heart consisting of waves called P, Q, R, S, T, and sometimes U. This test gives information concering the spread of electricity and is used to diagnose rhythm and conduction disturbances. (MI, ischemia, metabolic disorders)
2. Electrodes are placed on the patient's chest.
