Cards

Term
Trendellenberg sign
Definition
Pt standing - to raise 1 leg (good leg first), + for gleut med weakness or hip instability indicated by hip drop on opposite side
Term
Patrick/FABER/Jansen/Figure-4 test
Definition

Pt supine, test leg crossed over in fig-4 position;

If test leg cannot be pushed to parallel, or iliopsoas spasms, is + test.

-Ask WHERE pain is: groin indicates HIP problem, back/thigh indicates SI problem.

Term
Craig’s/Trochanteric Prominence test
Definition
-pt prone with knee flexed to 90˚
-PT palpates greater trochanter while rotating leg until trochanter is felt to reach most prominent position.
-Pt to hold position while PT measures the angle of the lower leg long axis with vs vertical (perpendicular to table).
-If >15˚, + for femoral anteversion.
-If <8˚, + for femoral retroversion.
Term
Leg Length Difference (LLD) tests - TRUE LLD
Definition
-pt prone with legs 4-8 inches apart; -Measure from ASIS on one side to medial malleolus on the same leg. Measure same thing on opposite side. -1-1.5cm difference is normal but can still cause symptoms.
Term
Leg Length Difference (LLD) tests - FUNCTIONAL
Definition
pt lies supine in neutral (pelvis may be laterally rotated if functional shortening present).  Pull legs to chest, then extend out. -Measure distance from umbilicis to med malleolus, compare sides.
Term
Squish Test (Approximation/Compression Test)
(Laslett cluster)
Definition
Pt S/L, apply compression pressure on top ilium toward table. Pain indicates + test for POSTERIOR SI lesion or sprain.
Term
Distraction/Gapping test (Laslett cluster)
Definition
Pt supine, cross arms and press iliums away from each other with hands. Unilateral gluteal or post leg pain indicates positive test for lesion of ANTERIOR SI ligaments
Term
SLR Test (as in LE screen)
Definition

• Pt in supine • PT passively flexes pt’s hip w/ slight IR and slight ADD.  Backs tension off, then DF the ankle. If discomfort doesn't return, indicates - test.

If discomfort returns, ask where.  Unless they say "calf" or "lower leg" is + for neural tightness.

Term
Supine-to-Sit Test
("Long-Sitting Test")
Definition
Pt lies supine with legs straight
PT ensures medial malleoli are level
Pt sits up , PT observes whether one leg is pulled proximally into the hip joint farther than the other; indicates + test for *functional* leg length difference
Term
Gaenslen test (Laslett cluster)
Definition

1. Pt is supine at edge of table 2. Pt brings both legs up to chest and slowly lowers the test leg into extension (hanging over edge of table) 3. PT applies OP to leg hanging over the table 4. Positive test for SI issue if elicits pain in low back region 5. Compare to other side

(also in flynn)

Term
P4 test/Thigh thrust test (Laslett cluster)
Definition
Pt supine, with knee that is farthest from PT bent. PT to grasp this leg with the arm nearest the pt's head, rock the pt sideways and slide other hand underneath the sacrum.
-Pt rolled back supine overtop of the PT hand
-PT thrusts compression through the bent knee, feels for posterior distraction
-Post distraction indicates + test for SI issue on that side
Term
Cephalad/Caudal movements of the Ilium/Sacrum
Definition
Pt prone; PT presses UP on the sacrum and DOWN on the ilium (or vice-versa). Compare L/R.
Term
Sacral Thrust (Laslett cluster)
Definition
Pt prone, depress sacrum toward table, both hands. -- a PA of the sacrum
Term

Length test - Piriformis

Definition

Pt supine in fig-4;is OPPOSITE of faber test, so pulling pts test leg TOWARD examiner, just test for tightness, if tightness reported is + test.

*be sure to palpate the piriformis too!

Term
Seated flexion test (flynn)
Definition
Have pt roll down waistline to make easier palpation, then sit on a stool; PT palpates PSIS, has pt lean fwd. Palpates sacral sulcus and ILA's.
Asymmetrical superior shift indicates + test for SI and suggests it is a SACRAL issue if it is worse than in standing.
Term
Slump Test (flynn)
Definition
Pt short-sitting, hands behind back, knees together.
PT w/ one arm/hand applying slight OP to upper back/neck, one hand grasping same-side foot and pulling into slight DF-
Motion sequence: Spine flex -> neck flex -> knee ext -> release neck flex
Sxs decrease w/ release of neck flexion indicates + test for neural sensitivity
Term
Prone Instability Test (Flynn)
Definition
o The patient lies prone with the body on the examining table and legs over the edge and feet resting on the floor
o PT applies PA pressure to L spine. Any pain is reported.
o At painful point, pt lifts legs up and compression is applied again
o Pain in resting position that goes away during legs up position Indicates + test for instability
Term
McKenzie Repetitive Mvmt:
Definition
1. Have baseline pain rating for all pain locations
2.??
3. Follow-up assessment of pain locations and pain ratings between movements and at the end.
Term
Laslet cluster - how many + do you need to confirm?
Definition
